北京市某三甲医院互联网诊疗开展情况分析

    背景 国家卫生政策建议各医疗机构要积极采取信息化手段,加强互联网 + 医疗服务,优化诊疗服务方式,强化互联网服务供给。
    目的 通过分析北京世纪坛医院互联网诊疗开展情况,找到适合进行互联网诊疗的群体,为公立医院互联网诊疗建设提供参考。
    方法 分析北京世纪坛医院2020年4月 - 2022年12月互联网诊疗开展情况。
    结果 通过分析,发现互联网诊疗就诊人次呈逐年递增的趋势,2022年较2021年全年环比增长了131.4%,其中妇科、产科、肿瘤科、变态反应科等特色科室诊次相对较高,合计占比89%;妇科诊次最多,占比59.2%。线上平均就诊时长9.25 min/人次,高于线下5.10 min/人次,表明线上诊疗过程中,接诊医师与患者交流的时间更长。选择互联网诊疗的患者中,以19 ~ 40岁年龄段患者居多,占比63.27%;女性患者占比90.62%;医保患者占比66.67%。除常规诊疗外,开具处方药占比13.70%,开具检查占比76.92%,说明预约检查是互联网诊疗中最常见的服务需求,药物配送方式中选择药品邮寄服务占比28.89%。
    结论 19 ~ 40岁的年轻人是互联网诊疗的主要就诊群体,发挥医院专业特色,加强医保服务力度,满足医保患者就诊需求,是未来提高互联网诊疗服务能力的努力方向。

     

    Abstract:
    Background The national health policy suggests that all medical institutions should actively adopt information technology, strengthen internet + medical services, optimize diagnosis and treatment services, and strengthen internet service supply.
    Objective To identify the appropriate group for online diagnosis and treatment, and provide reference for the construction of online diagnosis and treatment in public hospitals by analyzing the development of online diagnosis and treatment in Beijing Shijitan Hospital.
    Methods The processes of online diagnosis and treatment in Beijing Shijitan Hospital from 2020 to 2022 were analyzed.
    Results We found that the number of internet medical consultations increased year by year. In 2022, the total number of consultations increased by 131.4% compared to 2021, with relatively high number for specialty departments such as gynecology, obstetrics, oncology, and allergy, accounting for a total of 89%. Gynecological consultations were the highest, accounting for 59.2%. The average online consultation time was 9.25 minutes/person, higher than the offline consultation time (5.10 minutes/person), indicating that doctors spent more time in communicating with patients during online consultations. Among patients who chose internet medical consultations, the majority of patients aged 19-40 years old, accounting for 63.27%. Female patients accounted for 90.62% and patients with medical insurance accounted for 66.67%, both of which were relatively high. In addition to routine consultations, prescription medications accounted for 13.70%, testing accounted for 76.92%, and medication delivery services accounted for 28.89%, indicating that the demand for appointment scheduling for testing was the most common in internet medical consultations.
    Conclusion Young people aged 19-40 years are the main target group for online diagnosis and treatment. To improve online diagnosis and treatment services, hospitals should focus on their specialty, strengthen medical insurance services, expand their service scope, and meet the needs of medical insurance patients, and constantly adapt to the changing demands of online diagnosis and treatment services.
