石墨烯发热护腰治疗慢性非特异性下腰痛的疗效研究

Efficacy of a graphene heating waist belt in treatment of chronic non-specific low back pain

  • 摘要: 背景 慢性非特异性下腰痛(chronic non-specific low back pain,CNLBP)是临床高发的肌肉骨骼系统疾病,其病程较长、病理机制复杂。石墨烯柔性发热材料凭借其独特优势,为CNLBP的无创康复治疗提供了新手段。目的 初步探索石墨烯发热护腰对CNLBP的临床疗效。方法 采用前瞻性自身前后对照研究,选取2024 年5 月— 2025 年1 月于解放军总医院第一医学中心康复医学科就诊的CNLBP患者。所有患者均接受为期2 周的石墨烯发热护腰理疗干预。评估治疗前、治疗1 周后及治疗2 周后的疼痛程度、腰椎功能、心理状况、生活质量和肌肉生物力学等相关指标。采用重复测量方差分析比较各时间点指标差异,包括视觉模拟评分(Visual Analogue Scale,VAS)、简易McGill 疼痛问卷(Short-Form McGill Pain Questionnaire,SF-MPQ)、关节活动度(Range of Motion,ROM)、Oswestry 功能障碍指数(Oswestry Disability Index,ODI)、腰椎日本骨科协会评分(Japanese Orthopaedic Association Scores for Lumbar Spine,JOA)、恐惧回避信念问卷(Fear-Avoidance Beliefs Questionnaire,FABQ)、中枢敏化量表(Central Sensitization Inventory,CSI)、36 条简明健康状况调查表(36-ItemShort Form Survey,SF-36)、肌肉张力和杨氏模量等。结果 共纳入31 例患者,其中男28 例,女3 例;平均年龄(28.16±3.93)岁,平均病程2.29 年。治疗1 周后,运动和休息时的VAS评分、SF-MPQ评分、ODI评分、竖脊肌收缩位移和曲线下面积(area under the curve,AUC)较治疗前均显著降低(P<0.05),腰椎活动度、JOA评分、竖脊肌收缩与舒张面积差较治疗前均显著升高(P<0.05)。治疗2 周后,除肌张力相关指标外,上述指标在原有改善基础上进一步显著变化(P<0.05);FABQ、CSI 评分首次呈现显著改善(P<0.05);但SF-36 评分、竖脊肌舒张位移、竖脊肌舒张AUC和杨氏模量始终未呈现显著变化(P>0.05)。结论 石墨烯发热护腰在缓解CNLBP患者疼痛、改善腰部功能和心理状况等方面具有积极作用,但对患者短期生活质量及肌肉生物力学特性的改善有限。上述结果尚需更大规模、设计严格的研究进一步验证。

     

    Abstract: Background Chronic non-specific low back pain (CNLBP) is a highly prevalent musculoskeletal disorder in clinical practice, characterized by a long disease course and complex pathological mechanisms. Graphene-based flexible heating materials, with their unique advantages, have provided a new approach for the non-invasive rehabilitation treatment of CNLBP. Objective To preliminarily evaluate the clinical efficacy of the graphene heating waist belt in the treatment of CNLBP. Methods  This prospective self-controlled study enrolled consecutive CNLBP patients presenting to our institution from May 2024 to January 2025. All participants underwent a 2-week physiotherapeutic intervention with the graphene intelligent heating waist belt. Relevant indicators, including pain intensity, lumbar function, psychological status, quality of life, and muscular biomechanical parameters, were evaluated at baseline and at 1 and 2 weeks after treatment initiation. A repeated measures analysis of variance (ANOVA) was used to compare the differences in indicators across the various time points, including the Visual Analogue Scale (VAS), Short-Form McGill Pain Questionnaire (SF-MPQ), Range of Motion (ROM), Oswestry Disability Index (ODI), Japanese Orthopaedic Association Scores for Lumbar Spine (JOA), Fear-Avoidance Beliefs Questionnaire (FABQ), Central Sensitization Inventory (CSI), 36-Item Short Form Survey (SF-36), muscle tension, and Young's modulus. Results A total of 31 patients were enrolled, consisting of 28 males and 3 females, with a mean age of (28.16±3.93) years and a mean disease duration of 2.29 years. After 1 week of treatment, VAS scores (both during movement and at rest), SF-MPQ scores, ODI scores, and the contraction displacement and contraction area under the curve (AUC) of the erector spinae muscle decreased significantly (P<0.05), while lumbar ROM, JOA scores, and the area difference between contraction and relaxation of the erector spinae muscle increased significantly (P<0.05). After 2 weeks of treatment, except for muscle tone indices, the aforementioned measures showed further significant improvements consistent with the initial trend, and FABQ and CSI scores showed significant improvements for the first time (P<0.05). However, no significant changes were observed in SF-36 scores, erector spinae muscle relaxation displacement, relaxation AUC, or Young's modulus throughout the treatment period (P>0.05). Conclusion The graphene heating waist belt plays a positive role in pain relief, lumbar function improvement, and psychological status adjustment for patients with CNLBP, while its short-term effects on improving quality of life and modifying muscular biomechanical characteristics are limited. Further large-scale and rigorously designed clinical trials are required to verify these findings.
