护士视角下髋部骨折二次骨折预防护理实践差异性成因及改善对策的质性研究

A qualitative study on determinants of variability in nursing practices for secondary fracture prevention in elderly patients with hip fracture and strategies for improvement

  • 摘要: 背景 髋部脆性骨折患者二次骨折高发,严重影响患者预后并加剧医疗与家庭负担,现有循证预防证据在临床护理转化过程中落地不均衡,护士相关护理实践差异突出。目的 深入分析髋部骨折二次骨折预防护理实践差异性成因及改善需求与对策,为制定同质化护理实践方案提供切入点。方法 本研究采用目的抽样法,以信息饱和为原则,在全国范围内不同区域医疗机构共抽取15 名护理人员,并选取中华护理学会骨科专委会6 名专家进行半结构式访谈。使用Nvivo 15.0进行资料提取,采用COM-B理论引导下的定向内容分析为主框架,同时结合Colaizzi 现象学方法中的分析技术。结果 共形成机会、能力、动机3 个主题、11 个亚主题。机会层面包含专业知识与技能培训机会、可支配时间、学习资源、统一规范性指导、管理层支持、督导评估机制、个人价值体现的激励措施;能力层面包含临床知识与实践能力、自主学习能力;动机层面包含自身认知与重视程度、主动学习意愿。结论 未来应通过提供可及性髋部骨折护理方案创造机会,分层培训提升二次骨折预防护理实践能力,充分调动护理人员二次骨折预防干预动机,三者协同制定干预措施,以缩小护理实践差距,改善患者预后。

     

    Abstract: Background Hip fragility fractures are associated with a high incidence of subsequent fractures, leading to poor patient outcomes and increased medical and family costs. Currently, the uptake of evidence-based preventive measures in nursing practice is inconsistent, and considerable variations exist among nurses' clinical practices. Objective To explore the causes of heterogeneity in nursing practice for secondary hip fracture prevention and to identify improvement needs and strategies based on the COM‑B theoretical model, so as to provide entry points for developing standardized, homogeneous nursing practice protocols. Methods Purposive sampling was adopted in this study, with information saturation serving as the guiding principle for sample size determination. Semi-structured interviews were conducted with 15 nursing staff recruited from medical institutions in various regions across the country, as well as with 6 experts affiliated with the Orthopaedic Nursing Committee of the Chinese Nursing Association. Data extraction was facilitated using NVivo 15.0, and the analysis was primarily conducted through directed content analysis underpinned by the COM-B theoretical framework, while also incorporating analytic procedures from Colaizzi's phenomenological approach. Results Three main themes-opportunity, ability, and motivation-and 11 sub-themes were extracted. Opportunity-related factors included skills training, time availability, resources, standardized guidance, managerial support, supervisory evaluation, and value-based incentives. Ability-related factors comprised clinical competence and self-directed learning. Motivation-related factors involved self-awareness and learning initiative. Conclusion Future efforts should focus on creating opportunities through the provision of accessible hip fracture care protocols, enhancing nurses' secondary fracture prevention practice capabilities via stratified training, and strengthening motivation for secondary fracture prevention by refining incentive mechanisms. These three dimensions should be addressed synergistically through the development of tailored interventions, with the ultimate goal of narrowing the gap in nursing practice and improving patient outcomes.
