无创监测技术在尿毒症患者肾脏替代治疗中低血压防治的应用与研究进展

Advances in the application and research of non-invasive monitoring technology for hypotension prevention and management in renal replacement therapy for uremic patients

  • 摘要: 慢性肾脏病是全球重大公共卫生挑战,尿毒症期患者常需接受肾脏替代治疗,但治疗期间低血压发生率居高不 下,可导致重要器官灌注不足并增加死亡风险。传统以血压为中心的滞后管理模式亟需向主动预防转变。本文系统论述了 低血压的根源在于患者个体化生理失衡与血流动力学评估不精准。以生物电阻抗分析、在线血容量监测、下腔静脉超声及 无创心输出量监测为代表的无创血流动力学监测技术,为早期风险预警提供了关键工具。基于对现有证据的系统梳理,本 综述整合并提出了以精准容量评估为核心、贯穿治疗全程的护理干预框架,明确了护理在从“事后处置”转向“主动防控” 范式转变中的关键角色与实施路径,为未来的临床实践与研究提供了参考方向。

     

    Abstract: Chronic kidney disease represents a major global public health challenge.Patients in the uremic stage often require renal replacement therapy,however,the incidence of hypotension during treatment remains high,which can lead to hypoperfusion of vital organs and increase the risk of mortality.The traditional blood pressure-centered reactive management model urgently needs to shift toward proactive prevention.This review systematically elaborates that the root cause of hypotension lies in imprecise volume assessment and individualized physiological imbalances in patients. Non-invasive technologies, such as bioelectrical impedance analysis,online blood volume monitoring,inferior vena cava ultrasonography,and non-invasive cardiac output monitoring,provide key tools for early risk warning.Based on a systematic synthesis of existing evidence,this review integrates and proposes a theoretical framework for graded nursing intervention centered on precise volume assessment and spanning the entire treatment process.The framework clarifies the critical role and implementation pathway of nursing in the paradigm shift from "reactive management"to"proactive early warning and prevention",offering clear guidance for future clinical practice and research.
