邢台地区某煤矿工人牙体楔状缺损调查及病因分析

An investigation of the prevalence and etiology of wedge-shaped defects among coal miners in Xingtai, China

      背景  口腔疾病已经成为影响煤矿工人身心健康的常见病、多发病,目前尚缺乏煤矿工人口腔楔状缺损患病情况的相关调查研究。
      目的  了解邢台地区煤矿工人牙体楔状缺损情况,探讨楔状缺损发生的病因,为煤矿工人楔状缺损的预防提供依据。
      方法  2017年10月对邢台地区1 116名煤矿工人的牙体楔状缺损情况进行调查,按年龄、牙位分布、频数分布、缺损形状和程度、刷牙方式、夜磨牙情况、反酸情况进行统计分析。
      结果  1 116名25~55岁煤矿工人中,男性1 059名,女性57名,楔状缺损患病率44.35%。随年龄的增长,患病率逐渐增高,其中35岁以下为31.92%,35~44岁为43.63%,45岁以上57.63%。发病率最高的是第一前磨牙,月形缺损达85.52%,1度缺损达74.14%。拉锯式刷牙楔状缺损患病率最高,达53.93%。夜磨牙工人楔状缺损患病率达86.67%;有胃食管反流病史患者楔状缺损患病率高达75.00%。
      结论  矿工牙体楔状缺损与年龄、拉锯式刷牙、夜磨牙和胃食管反流有关。多数工人为月形和1度缺损,可通过早期干预,如纠正刷牙方式为Bass刷牙法、合垫纠正夜磨牙、胃反酸治疗等阻止楔状缺损进一步发展。

     

    Abstract:
      Background  Oral diseases have become a common and frequently occurring disease affecting the physical and mental health of coal miners, but there is still a lack of relevant investigation and research on the prevalence of teeth wedge-shaped defects in coal miners.
      Objective  To investigate the prevalence and etiology of wedge-shaped defects in coal miners in Xingtai, China, and to provide evidence for the prevention of wedge-shaped defects in coal miners.
      Methods  An investigation of wedge-shaped defects was performed for 1116 coal miners in Xingtai, and a statistical analysis was performed for age, tooth position, frequency distribution, shape and degree of defects, toothbrushing method, bruxism, and gastroesophageal reflux (GER).
      Results  Among the 1116 coal miners aged 25-55 years, there were 1059 male workers and 57 female workers. The prevalence rate of wedge-shaped defects was 44.35%, and gradually increased with age, with 31.92% in the workers aged <35 years, 43.63% in the workers aged 35-44 years, and 57.63% in the workers aged >45 years. Moreover, the highest prevalence rate was observed in the first premolar, and the prevalence rates of moon-shaped defect and grade 1 defect were 85.52% and 74.14%, respectively. The workers who brushed their teeth in a sawing motion had the highest prevalence rate of wedge-shaped defects (53.93%). The prevalence rate of wedge-shaped defects reached 86.67% in workers with bruxism and 75.00% in workers with a history of GER.
      Conclusion  Wedge-shaped defects in miners are associated with multiple factors such as age, toothbrushing in a sawing motion, bruxism, and GER. Most workers have moon-shaped defect and grade 1 defect. Early intervention, such as changing toothbrushing method to Bass method, treating bruxism with occlusal splint, and managing GER, can prevent the progression of wedge-shaped defects.
