Efficacy of underlay and over lay techniques in repairing large per foration of tym panic membrane

Abstract
Objective Toexplore the efficacy of underlay and overlay techniques of UgoFisch's myringoplasty in repairing large perforation of tympanic membrane. Methods Clinical data about 58 cases (60 ears) with subtotal perforation tympanic membrane whounderwent UgoFisch surgery in our department from January 2012 to June 2013 were retrospectively analyzed. Underlay and overlay techniques were usedduring operation with a slit cut in the graft toaccommodate the hand le of malleus. The post-operative graft integration rates, the hearing results and complications were analyzed. Results The 1-2 year follow-uprevealedthat 57 ears recoveredat stage Ⅰ with a satisfactory hearing enhancement. The overall perforation closure rate was 95%. The mean postoperative air conduction thresholdin langue frequency was 21.53±6.98 dB HL with air-bone gapof 13.78±7.32 dB. There was statistically significant improvement in air conduction thresholds after myringoplasty (P< 0.05). The post-operative bone conduction thresholdwas similar topre-operative, and nocomplications of tinnitus or aggravation and facial nerve palsies were found. Conclusion Underlay and overlay techniques of UgoFisch's myringoplasty is an ideal methodfor repairing subtotal perforation of tympanic memb rane.
