Value of albumin in predicting prognosis of acute pulmonary embolism

Abstract
Objective To investigate the changes of albumin (ALB) in patients with acute pulmonary embolism (APE) and its prognostic value. Methods From August 2013 to August 2019,clinical data about 229 patients with APE admitted to the Affiliated Hospital of Southwest Medical University were collected.According to the prognosis of patients,they were divided into death group and survival group.ROC curve was used to analyze the predictive value of serum albumin level for the prognosis of APE patients.Pearson correlation test was used to analyze the correlation between serum albumin level and risk stratification or simplified Pulmonary Embolism Severity Index score (sPESI score). Results The 3-month all-cause mortality rate of APE patients was 20.96%.The serum albumin in the death group was significantly lower than that in the survival group (ALBg/L:4.36±4.44 vs 39.58±4.06,P=00.000).Risk stratification by European Society of Cardiology guideline (OR:3.145;95% CI:1.560-6.340),age (OR:1.035;95% CI:1.006-1.065),sPESI score (OR:2.750;95% CI:1.282-5.899),High-cTnI/T (OR:3.361;95% CI:1.912-5.907),BNP (OR:4.648;95% CI:2.076-10.407),RVD (OR:3.571;95% CI:1.780-7.164),SaO2 less than 90% (OR:4.410;95% CI:1.640-11.862) and serum albumin (OR:0.747;95% CI:0.642-0.869) were the prognostic factors for APE.The AUC of age in predicting early death was 0.628 (95% CI:0.562-0.691),with sensitivity of 0.687 and specificity of 0.547;The AUC of serum albumin was 0.827 (95% CI:0.114-0.231),with sensitivity of 0.813 and specificity of 0.680;The AUC of combined application of age and serum albumin was 0.834 (95% CI:0.779-0.879),with sensitivity of 0.821 and specificity of 0.790. Conclusion Serum albumin level is related to the severity and prognosis of APE patients,which has certain predictive value for the prognosis of APE.
