Correlation between obesity and UACR in middle-aged and elderly people in China

Objective To investigate the correlation between obesity and urinary albumin creatinine ratio in people over 40 years old in China. Methods Totally 41085 cases without chronic kidney disease （CKD） were selected from the REACTION study in this research.Obesity measure included body mass index （BMI≥28.0 kg/m²） and waist-hip ratio （WHR≥0.9 in male or≥0.85 in female）,and then the samples were divided into non-obese,peripheral obese,central obese,peripheral and central obese groups.Level of urinary albumin creatinine ratio （UACR） was transformed into ordinal data by the first,second and third quantile.Ordinary logistic regression was used to investigate the relationship between obesity and UACR,and the discrepancy in subgroups of population （by gender,age,blood pressure,blood lipid,glucose metabolism status）. Results The incidence of hypertension,diabetes,coronary heart disease,hyperlipidemia,and the level of TG,SBP,DBP,and blood glucose in obesity people were higher than those in normal weight group （all P<0.01）.People in peripheral and central obese groups had a higher risk of elevated UACR （OR:1.14;95% CI:1.07-1.12）,even after adjusted for multiple factors.In subgroup analysis,the risk of elevated UACR was higher in male （OR:1.25;95% CI:1.12-1.39）,in cases with hypertension （OR:1.27;95% CI:1.16-1.40）,HbA1c≥6.5% （OR:1.14;95% CI:1.06-1.38）,age≥60 years old （OR:1.22;95% CI:1.13-1.37）. Conclusion In China,people with central and peripheral obesity have higher risk of elevated UACR.The risk of UACR in male population,and people with hypertension,advanced age,high glycated hemoglobin is further increased,which suggests that people with these factors need to pay more attention to weight control for the prevention of CKD.
