First aid of severe multiple traumas

  • This paper reports the experiences in the firest-aid of serious multiple traumas. A total of 107 cases of severe multiple traumas were admited in the past 10 years, of whom 27 patients died in the emergency service. Based on the fairly energic treatment in these cases experences may be summed up as follows: 1. Surgeons in the emergency service have to know thoroughly the nature and the seriousness of multiple traumas, and the technique of first aid to rescue as many patients as possible. 2. The traumatic shock should be rapidly managed with replacement of balanced electrolyte solution and matched whole blood, or even "O" type blood without crossmeching, if necessary. 3. The traumas should be handled strictly according to the rule of first aid priorities, the general order being: thoracic trauma. abdominal one, craniocerebral one, genitourinary, and extremity one. 4. In critical cases of severe shock with serious intrathoracic injuries, intraabdominal hemorrhage of craniocerebral hematoma, for saying the life of patients rapidly, emergency thoracotomy. abdominal exploration, or craniotomy. as indicated, should be done in the emergency room. 5. ICU should be established in the emergency room. where traumatic patients with impairment of vital mechanisms are monitored and rescued as effectively as possible
