Survey and analysis on the prescriptions for hospital discharge patients

Objective: To investigate and analyze the errors in the prescription of hospital discharge patients and to put forword the corresponding measures.Methods: From August 2006 to August 2008,a total of 37 121 prescriptions of leaving hospital patients were taken for the survey and analysis of errors.Results: In the total prescriptions,3 906（10.52%）were irrational,in which 419（1.13%）were related with drug.The main problems were the standardization of the prescription,3 406（87.7%） were erroneous in these way.Conclusion: Clinicians and pharmacist should make more efforts to learn"prescription management method" and expertise and to enhance the ability of writing and checking prescription.
