老年糖尿病患者胰岛素笔院外使用现状分析

摘要: 目的 了解老年糖尿病患者胰岛素笔院外使用现状,提高胰岛素笔院外使用的正确率,为进行针对性的健康教育提供依据。 方法 使用自行设计问卷,调查2011年1-6月在我科住院糖尿病患者180例院外胰岛素笔的使用现状。选取60例住院糖尿病患者进行预实验,得到问卷的内在一致性信度Cronbach'sα系数为0.89,内容效度为0.94。结果采用率进行描述。 结果 胰岛素注射前环节中,每次注射前洗手的为20%,36.11%会选择碘剂作为消毒剂,知晓每次注射前排气并执行的为2.78%;胰岛素注射过程中,有规律地和避开脐周5cm选择注射部位知晓率分别为43.33%和37.78%,非握拳式持笔姿势的为40%;胰岛素注射后环节中,每次用后更换胰岛素笔针头的正确率为0,注射完毕6s后拔出,并按压注射键的正确率为21.11%,注射后用棉签轻拭注射点的正确率为25%,废弃的针头放在锐器盒中到指定地点丢弃的正确率为13.33%;胰岛素笔使用常识中,开启后的胰岛素笔存放于室温、避光阴凉处知晓率为25%,开启后的胰岛素笔有效期限知晓率为27.22%。 结论 胰岛素笔院外使用现状并不乐观,应加强院外自我胰岛素注射患者的健康教育。Abstract: Objective To provide the evidence for health education of elderly diabetes mellitus(DM) patients by investigating their use of insulin pens outside of hospital. Methods Use of insulin pens in 60 DM patients discharged from hospital after they were treated from January 2011 to June 2011 was investigated using questionnaire we designed.The credibility of Cronbach’sαcoefficient was 0.89 and the content validity was 0.94 obtained from the returned questionnaires.The results were described by rate. Results Only 20% patients washed their hands before insulin injection,36.11% patients chose iodine preparation as their disinfectant,only 2.78% patients exhausted the air in injectors before injection,only 43.33% and 37.78% patients selected the injection sites 5cm around the umbilicus and avoid Cullen during insulin injection,40% patients injected insulin in a non-fist holding posture,no patient replaced the insulin pen needle after injection,only 21.11% patients pulled out the needle 6s after injection and pressed the injection button,only 13.33% put needles into the sharp box and discarded them at specified locations,only 25% patients stored the insulin pen in a dark place at room temperature,27.22% knew the expiry date of opened insulin pens. Conclusion The use of insulin pens in DM patients outside of hospital is not optimistic and it is thus necessary to strengthen health education for those who inject insulin themselves.
