探讨应用辅助体表标记技术降低鼻咽癌放疗摆位误差的有效性

Auxiliary surface markers technique in reducing setup errors for patients with nasopharyngeal carcinoma during radiotherapy

  • 摘要: 目的 评估辅助体表标记技术(auxiliary surface markers,ASM)降低鼻咽癌放疗摆位误差的可行性。方法 选取2017年2月-2018年2月解放军总医院海南医院收治的12例采用调强放疗的初治鼻咽癌患者。按照摆位方法进行分组,奇数次摆位采用单等中心摆位法(single isocenter method,SIM)和ASM,偶数次摆位仅采用SIM。每次行锥形束CT(cone beamcomputed tomography,CBCT)扫描及图像配准,分别记录线性方向误差X、Y、Z及旋转方向误差Rx、Ry、Rz。结果 SIM联合ASM与SIM法线性误差比较,X轴:(0.14±0.11) cm vs (0.16±0.12) cm,P=0.044;Y轴:(0.16±0.10) cm vs (0.17±0.10) cm,P=0.218;Z轴:(0.14±0.11) cm vs (0.14±0.10) cm,P=0.615;而旋转误差方面,ASM的Rx、Ry、Rz分别下降了0.05°、0.15°和0.22°,Ry及Rz方向差异存在统计学意义(P=0.006、0.000)。结论 在鼻咽癌放疗摆位中,应用ASM可以有效地降低X轴线性误差、Ry及Rz旋转误差,且操作简单易行,临床上值得应用。

     

    Abstract: Objective To evaluate the effects of auxiliary surface markers technique(ASM) in reducing setup errors in radiotherapy for patients with nasopharyngeal carcinoma(NPC). Methods Twelve newly diagnosed NPC patients treated by intensity-modulated radiation therapy(IMRT) were enrolled in the study. Positioning should be aligned both with isocenter and ASM lines at odd number of times, while only with isocenter at even number of times. Through cone beam computed tomography(CBCT) scans and image registration, the translational errors of X, Y, Z and the rotational errors of Rx, Ry, Rz were recorded respectively. Results The translational errors of X, Y, Z were(0.14±0.14) cm,(0.16±0.10) cm,(0.14±0.11) cm by ASM and(0.16±0.12) cm,(0.17±0.10) cm,(0.14±0.10) cm by single isocenter method(SIM), respectively, with statistically significant difference in X direction between two methods(HR=0.044). The rotational errors of Rx, Ry and Rz were(0.35°±0.33°),(0.77°±0.64°),(0.44°±0.42°) by ASM and(0.41°±0.36°),(0.92°±0.63°),(0.74°±0.52°) by SIM, respectively. The differences in Ry and Rz between two method were statistically significant(HR=0.006, 0.000). Conclusion Using ASM, the translational errors of X axis, and the rotational errors of Ry and Rz can be effectively reduced for NPC patients during daily positioning in IMRT, which is worthy of application in the clinical practice.
