某军队医院医务人员焦虑状况调查及影响因素分析

Investigation and analysis of anxiety status of military medical personnel

  • 摘要: 目的 通过调查某军队医院医务人员的焦虑现状,分析影响军队医务人员焦虑水平的相关影响因素。方法 采用随机抽样方法,2016年2-4月抽取某三级甲等军队医院322名医务人员,包含军人(181,56.21%)、非现役(18,5.59%)和聘用人员(123,38.20%),其中医生120人(37.27%)、护士145人(45.03%),医技系列57人(17.70%),使用焦虑自评量表SAS评定不同医务人员的焦虑状况,应用t检验和F检验对焦虑总分与相关因素进行分析。结果 322名军队医务人员焦虑的发生率为24.8%,焦虑自评量表总分为43.62±10.68,高于国内常模的29.78±0.46(P<0.001)。单因素分析结果显示,不同性别、职务身份、职业类别、文化程度以及单位是否提供住宿情况对医务人员的焦虑总分没有影响(P> 0.05)。而不同婚姻状况、周工作时长、上夜班情况、参与抢救病人频率、接触传染病病人情况、与领导沟通频率以及购买保险情况对医务人员焦虑总分有显著影响(P均<0.05)。Logistic回归分析结果显示,较长周工作时间(OR=1.971,P=0.041)、高频率接触传染病病人(OR=1.618,P=0.005)是发生焦虑的危险因素,积极与领导沟通者(OR=0.476,P<0.001)是发生焦虑的保护因素。结论 军队医务人员焦虑水平显著高于一般人群。合理安排工作时间、关注特殊岗位人群、加强上下级沟通可能是减少军队医务人员发生焦虑的有效途径。

     

    Abstract: Objective To investigate the anxiety status of medical staff in a military hospital and analyze its risk factors. Methods FromFebruary to April in 2016, 322 medical staff in a military hospital were selected, and the anxiety status of different medical staff was assessed by self-rating Anxiety Scale(SAS). t test and F test were used to analyze the total anxiety score and the related factors. Results The incidence of anxiety in 322 military medical staff was 24.8%, and the total score of SAS was significantly higher than that of the domestic norm(43.62±10.68 vs 29.78±0.46, P< 0.001). The results of univariate analysis showed that there was no significant difference in the total score of anxiety between different military medical staff by gender, job status, occupational category, education level and accommodation(all P> 0.05). However, there were statistically significant differences in anxiety by marital status, work week length, night shift, frequency of rescuing patients, frequency of exposure to infectious diseases, frequency of communication with leadership, and insurance coverage(P< 0.05, respectively). Logistic regression analysis showed that greater work week length(OR=1.971, P=0.041) and more frequent to exposure to infectious diseases(OR =1.618, P=0.005) were the risk factors for anxiety among medical staff. While more frequent communication with leaders(OR=0.476, P< 0.001) was the protective factor. Conclusion The anxiety level of military medical personnel is significantly higher than that of the general population. Reasonable arrangement of working hours, paying more attention to people frequently exposed to occupational hazards and strengthening communication between superiors and subordinates may be effective ways to reduce the anxiety of military medical personnel.
