高海拔地区部队官兵胃肠应激调查与内镜学评价

Perceived gastrointestinal stress in servicemen deployed in high-altitude area and their endoscopic evaluation

  • 摘要: 目的 了解高海拔地区部队官兵胃肠应激反应发生率、相关理化指标和胃镜下的黏膜评价,为高原胃肠应激防控提供依据。方法 2018年1月选取格尔木(377人,平均海拔3 000 m)、玉树(237人,平均海拔4 200 m)地区共614名部队官兵,通过自制《部队官兵胃肠流行病学问卷调查表》进行流行病学调查,现场检测血清幽门螺杆菌(Helicobacter pylori,Hp)抗体、便隐血,分析胃肠症状与Hp感染、便隐血的相关性及对有胃肠应激反应的官兵进行便携式一体化胃镜检查。结果 格尔木、玉树两地官兵胃肠应激反应发生率高达50.8%(312/614),其中以腹泻38.4%(236/614)、食欲减退35%(215/614)、腹胀27.9%(171/614)、腹痛17.6%(108/614)、口臭17.6%(108/614)、呕血/便血7.3%(45/614)多见。格尔木地区官兵腹泻44.6% (168/377) vs 28.7%(68/237)、口臭21.5%(81/377) vs 11.4%(27/237)、呕血/便血9.3%(35/377) vs 4.2%(10/237)发生率均高于玉树地区,差异均有统计学意义(P均<0.05)。两地官兵现症Hp平均感染率为49.8%(360/614),其中格尔木地区为49.3%(186/377),玉树地区为50.6%(120/237),差异无统计学意义。便隐血检出率为24.9%(153/614),其中格尔木地区官兵为15.4%(58/377),显著低于玉树地区的40.1%(95/237)(P<0.001)。其中212名有胃肠应激症状官兵行胃镜检查发现,均罹患不同程度的胃食管、十二指肠炎性改变,以胃炎、溃疡、黏膜糜烂、出血多见。结论 高海拔地区胃肠应激反应发生率高,可引起食欲缺乏、慢性腹胀、腹泻、腹痛等,应引起足够重视,积极探索有效的防治措施,提高高原卫勤保障能力。

     

    Abstract: Objective To investigate the incidence of gastrointestinal stress and conduct endoscopic mucosal evaluation to servicemen deployed in high-altitude areas. Methods In January 2018,a total of 614 servicemen were drawn from troops deployed in the area of Golmud (377 cases at the average altitude of 3 000 m) and Yushu (237 cases at the average altitude of 4 200 m).The epidemiological investigation was conducted by self-made questionnaire about gastroenterological symptoms.The serum helicobacter pylori (Hp) antibody and fecal occult blood were detected in these cohorts.The correlation between gastrointestinal symptoms,Hp infection and fecal occult blood was analyzed,and portable integrated digestive endoscopy was performed on soldiers with gastrointestinal stress to evaluate the mucosa changes. Results The incidence of gastrointestinal stress was as high as 50.8% (312/614),including diarrhea (38.4%,236/614),anorexia (35%,215/614),abdominal distension (27.9%,171/614),abdominal pain (17.6%,108/614),halitosis (17.6%,108/614),and hematemesis (7.3%,45/614).The incidences of diarrhea (44.6% vs 28.7%),ozostomia (21.5% vs 11.4%) and hematemesis/bleeding (9.3% vs 4.2%) in Golmud troops were significantly higher than those in Yushu (P<0.05,respectively).The average infection rate of Hp was 49.8% (360/614),with 49.3% (186/377) in Golmud and 50.6% (120/237) in Yushu,respectively,but no statistically significant difference was found between them.The detection rate of fecal occult blood was 24.9% (153/614),with 15.4% (58/377) in Golmud and 40.1% (95/237) in Yushu,showing a significant difference (P<0.001).Moreover,212 soldiers suffered with gastrointestinal stress symptoms received gastroscopy examination,and we found that all soldiers had different degrees of gastroesophageal and duodenal inflammations,including gastritis,ulcer,mucosal erosion and bleeding. Conclusion The incidence of gastrointestinal stress in troops deployed in high-altitude is high,such as diarrhea,abdominal distension,abdominal pain and constipation.We should positively explore and establish more effective methods to detect,prevent and manage the gastrointestinal health of soldiers.
