探讨关节液可溶性CD64诊断关节置换术后假体周围感染的价值

Diagnostic value of soluble CD64 in synovial fluid for periprosthetic joint infection

  • 摘要: 目的 探讨关节液可溶性CD64在关节置换术后假体周围感染诊断中的价值。方法 选取2017年12月-2018年9月在我科因关节假体周围感染或关节假体无菌性松动进行髋/膝关节翻修的40例患者。使用MSIS2011感染诊断标准,诊断为关节假体周围感染20例,非感染对照组20例。术前行关节穿刺留取患者关节液,使用ELISA法检测关节液可溶性CD64浓度,比较两组差异,使用受试者工作特征曲线分析关节液可溶性CD64在关节假体周围感染中的诊断效能。结果 在关节假体周围感染组关节液可溶性CD64浓度平均值为12.56μg/L,非感染组为10.79μg/L,两组差异无统计学意义(P>0.05)。绘制关节液可溶性CD64预测关节假体周围感染的ROC曲线(曲线下面积为0.642),cut-off值为10.145μg/L,对应的敏感度为65.2%,特异度为63.4%,阳性预测值为64.0%,阴性预测值为64.6%。结论 关节液可溶性CD64可以作为髋/膝关节置换术后假体周围感染的辅助检测手段,但敏感性较低,尚不足以作为一种术前筛查指标。

     

    Abstract: Objective To explore the role of synovial fluid soluble CD64 in the diagnosis of periprosthetic infection after joint replacement. Methods From December 2017 to September 2018,40 patients undergoing hip or knee revision due to joint infection or aseptic loosen admitted to our department were enrolled in our study.According to MSIS 2011 infection diagnostic criteria,20 cases were included in the periprosthetic joint infection group and 20 cases were in the non-infection group.The synovial fluid was taken by joint puncture before surgery,then the concentration of soluble CD64 in the synovial fluid was detected by ELISA,the difference between the two groups was compared,and the diagnostic efficacy of soluble CD64 in the synovial fluid for periprosthetic joint infection was analyzed by receiver operating curve. Results The mean concentration of soluble CD64 in synovial fluid in the periprosthetic joint infection group was 12.56μg/L,and it was 10.79μg/L in the non-infection group,without significant difference (P=0.288).The ROC curve of the synovial fluid soluble CD64 for predicting periprosthetic joint infection was obtained,and the area under the ROC curve (AUC) was 0.642,with 95% CI of 0.438 to 0.846 (P=0.180).The optimal cut-off value of soluble CD64 in synovial fluid was 10.145μg/L,with sensitivity of 65.2%,specificity of 63.4%,positive predictive value of 64.0%,and negative predictive value of 64.6%. Conclusion Soluble CD64 in synovial fluid has certain significance in the diagnosis of periprosthetic joint infection,but it can't be used as a preoperative screening indicator since its sensitivity is relatively low.
