人工全髋关节置换术术前应用模板测量软件预测假体选择的准确率探讨

Accuracy of digital templating in total hip arthroplasty:Acetabular component template vs femoral component template

  • 摘要: 目的 评估模板测量结果与术中实际使用假体型号大小的差异,确定可接受的模板测量结果误差范围,并对可能引起测量差异的原因进行分析。方法 分析我中心2018年3月-2019年3月收治的125例行人工全髋关节置换术患者利用计算机模板测量软件测定的假体型号与术中实际使用型号大小的差异。结果 髋臼侧假体测量值与实际值误差大部分在4 mm以内,股骨侧假体测量值与实际值误差大部分在3 mm以内。在相同的误差范围内,髋臼侧假体与股骨侧假体测量吻合率有统计学差异(误差4 mm以内:92.8% vs 99.2%,P<0.05)。此外,在髋臼侧假体测量吻合率方面,股骨头坏死患者明显优于成人发育性髋关节不良患者(P<0.05)。结论 应用模板测量软件辅助假体选择在一定误差范围内具有较高准确性和可信度,髋臼侧假体的测量准确率稍差于股骨侧假体,特殊的解剖学异常可能是引起测量差异的原因。

     

    Abstract: Objective To evaluate the difference between acetabular component template and femoral component template,and analyze the possible causes of the measurement differences. Methods From March 2018 to March 2019,125 patients admitted to our center who underwent total hip arthroplasty were selected.The difference between the prosthesis size measured by the computer template measurement software and the actual size used in the operation was retrospectively analyzed. Results The digital templating predicted the size of the prosthetic implant accurately with difference less than 4mm for the acetabular components,and less than 3mm for the femoral components.In terms of degree of difference,planning by acetabular component template was significantly more inaccurate than femoral component template (error within 4 mm,7.2% vs 0.8%,P<0.05).In addition,planning was more accurate in patients with femoral head necrosis than in patients with hip dysplasia (error within 4 mm,2 cases vs 7 cases,P<0.05). Conclusion The application of template measurement software to assist the selection of prosthesis has a high accuracy and validity within a certain error range.The measurement accuracy of the acetabular prosthesis is slightly lower than that of the femoral prosthesis.Specific anatomic abnomalies may be responsible for the difference in measurements.
