青光眼与白内障患者疾病不确定感、焦虑和抑郁状况及其影响因素分析

Comparison of uncertainty in illness,anxiety and depression in patients with glaucoma versus patients with cataract and their related influencing factors

  • 摘要: 目的 比较青光眼和白内障患者的疾病不确定感及焦虑抑郁评分,探讨其相关影响因素。方法 本研究调查2014年1月-2018年6月于北京丰台医院门诊就诊的原发性青光眼患者160例(平均年龄59.20岁)及白内障患者150例(平均年龄60.32岁),所有被调查者均填写包括基本信息的一般资料问卷、疾病不确定感量表(Mishel uncertainty in illness scale,MUIS)和综合医院焦虑抑郁自评量表(hospital anxiety and depression scale,HADS)。结果 青光眼患者的MUIS评分(90.37±12.66 vs 83.29±11.25)和HADS评分(12.16±7.81 vs 8.43±5.29)高于白内障患者(P均<0.05)。多元logistic回归分析结果显示:青光眼患者的疾病不确定感与HADS评分显著相关(OR=1.37,95% CI:1.31 ~ 1.43);焦虑情绪与健侧眼视力状况(OR=0.36,95% CI:0.15 ~ 0.82)及疾病不确定感(OR=1.07,95% CI:1.01 ~ 1.13)显著相关;抑郁情绪与教育水平显著相关(OR=0.54,95% CI:0.27 ~ 0.86)。白内障患者疾病不确定感与HADS-a评分显著相关(OR=1.28,95% CI:1.13 ~ 1.46);焦虑情绪与健侧眼视力(OR=0.33,95% CI:0.18 ~ 0.69)及患侧眼视力显著相关(OR=1.22,95% CI:1.06 ~ 1.44);抑郁情绪与MUIS评分显著相关(OR=3.02,95% CI:1.00 ~ 9.12)。结论 青光眼患者疾病不确定感及焦虑、抑郁发生率高于白内障患者。

     

    Abstract: Objective To compare the disease uncertainty,anxiety and depression scale in patients with glaucoma versus patients with cataract,and explore their related influencing factors. Methods One hundred and sixty patients (average age of 59.20 years) with primary glaucoma and 150 patients (average age of 60.32 years) with cataracts were recruited sequentially in Beijing Fengtai Hospital from January 2014 to June 2018.All the participants completed the questionnaires for socio-demographic information,Mishel uncertainty in illness scale (MUIS) and hospital anxiety and depression scale (HADS). Results The MUIS and HADS scores of glaucoma patients were significantly higher than those of cataract patients (MUIS,90.37±12.66 vs 83.29±11.25;HADS,12.16±7.81 vs 8.43±5.29;P<0.05,respectively).The results of multivariate logistic regression analysis showed that the disease uncertainty of glaucoma patients was significantly correlated with HADS score (OR=1.37,95% CI,1.31 to 1.43).Anxiety was significantly correlated with the visual acuity (VA) of healthy lateral eyes (OR=0.36,95% CI,0.15 to 0.82) and MUIS score (OR=1.07,95% CI,1.01-1.13).Depression was significantly correlated with educational level (OR=0.54,95% CI,0.27 to 0.86).The sense of uncertainty in illness in cataract patients was significantly correlated with HADS-a score (OR=1.28,95% CI,1.13 to 1.46).Anxiety was significantly correlated with the VA of the healthy eye (OR=0.33,95% CI,0.18 to 0.69) and VA of the eye with cataract (OR=1.22,95% CI,1.06 to 1.44),Depression was significantly correlated with MUIS score (OR=3.02,95% CI,1.00 to 9.12). Conclusion The incidence of uncertainty in illness,anxiety and depression in glaucoma patients is higher than that in cataract patients.
