肌氧监测指导下肢抗阻训练对脑卒中偏瘫患者下肢力量恢复的作用

Effects of muscle oxygen-guided resistance training on lower limb strength recovery in stroke patients with hemiplegia

  • 摘要: 目的 探讨肌氧监测指导下的抗阻训练方法对恢复期脑卒中偏瘫患者下肢力量及运动功能的影响。方法 将2019年3-9月解放军总医院第一医学中心康复医学中心处于脑卒中恢复期需要加强患侧下肢肌群力量的患者30名,随机分为常规组(n=15)和肌氧组(n=15)。两组均接受以Bobath神经促进疗法为主的常规康复治疗。在此基础上常规组利用下蹲肌群运动控制训练系统(MRS-FS)进行标准化的下肢半仰卧深蹲渐进抗阻训练;肌氧组以患侧股外侧肌氧饱和度曲线为训练强度参考,进行相同抗阻训练,为期4周,每周5次,每次15 ~ 20 min。训练前后均进行等速屈伸膝肌力、膝关节主动与被动关节活动度、"起立-行走"时间、10 m步行时间、Fugl-Meyer下肢功能评分、改良Ashworth量表等临床评价测试。结果 训练前两组患者一般资料及临床各项指标无统计学差异(P>0.05)。4周训练后,两组患者伸膝肌群峰力矩(peak torque,PT)、峰力矩体质量比(peak torque/body weight,PT/BW)、平均峰力矩(average peak torque,APT)在60°/s、120°/s和180°/s下均有显著改善(P均<0.05),且膝关节主动活动度、60°/s下屈伸膝峰力矩比率(flexor/extenson,F/E)、步行能力、Fugl-Meyer评分、MAS分级也有显著改善(P均<0.05),但被动活动度无差异。肌氧组60°/s、120°/s和180°/s下伸膝肌群PT、PT/BW、APT提升幅度较常规组更为明显(P均<0.05),但两组训练后屈膝肌力增长幅度以及其他指标的改善幅度无统计学差异(P>0.05)。结论 肌氧监测下抗阻训练方式对提高卒中偏瘫患者局部力量可能有益。

     

    Abstract: Objective To explore the effects of resistance training method under the guidance of muscle oxygen monitoring on lower limb strength and motor function in stroke patients with hemiplegia. Methods From March to September in 2019,30 patients in the rehabilitation department of the first medical center of Chinese PLA General Hospital who needed to improve the muscle strength of the affected lower limbs during the recovery period of stroke were randomly divided into routine group (n=15) and myo-oxygenation group (n=15).Both groups received the routine rehabilitation treatment mainly based on Bobath neurotherapy.On this basis,the routine group used MRS-FS to carry out the standardized leg semi supine squat progressive resistance training;the muscle oxygen group carried out the same resistance training under the guidance of the oxygen saturation curve of the affected lateral femoral muscle for 4 weeks,5 times a week,15-20 minutes each time.Before and after the training,the isokinetic flexion and extension of the knee,the active and passive joint activity of the knee,"stand to walk" time,10 meter walking time,Fugl Meyer lower extremity function score,the modified Ashworth scale and other clinical evaluation tests were carried out. Results There was no significant difference in general data and clinical characteristics between the two groups before training (all P>0.05).After 4 weeks of training,in both of the two groups,there were significant improvements in peak torque (PT),peak torque/body weight (PT/BW),average peak torque (APT) of keen extensors at 60°/s,120°/s and 180°/s angular velocity,and the knee joint active mobility,flexion/extension (F/E) at 60°/s,walking ability,Fugl-Meyer score,MAS (all P<0.05),but no significant difference was found in passive activity indexes.Compared with the routine group,myo-oxygenation group had significant greater increase in PT,PT/BW and APT of the lower extensor knee muscle (all P<0.05),but the knee flexion strength and other indexes after training showed no intergroup difference (all P>0.05). Conclusion It may be beneficial to improve the local strength of lower limb in stroke patients with hemiplegia by resistance training under muscle oxygen monitoring.
