脑膜败血伊丽莎白菌耐药性及同源性分析

Analysis of drug resistance and homology of Elizabethkingia meningoseptica

  • 摘要: 目的 分析我中心分离的脑膜败血伊丽莎白菌(Elizabethkingia meningoseptica,EME)的临床分布、耐药特征及同源性。方法 收集我中心2012年4月-2018年12月44例脑膜败血伊丽莎白菌感染患者的临床资料,采用质谱仪对44株菌进行再鉴定和同源性分析,用微量肉汤稀释法检测脑膜败血伊丽莎白菌对12种抗菌药物的耐药率。结果 44株脑膜败血伊丽莎白菌菌株标本主要来自患者下呼吸道(占81.7%),主要分布在ICU病房,其次为呼吸科和急诊科;这些菌株的相似程度低,说明EME在我中心各科室间没有引起传播流行。脑膜败血伊丽莎白菌对阿米卡星、环丙沙星、磺胺甲噁唑/甲氧苄啶和美罗培南耐药率均高于80%,对头孢他啶、头孢吡肟、庆大霉素的耐药率高达100%,而对米诺环素100%敏感。结论 44株脑膜败血伊丽莎白菌有较高的耐药水平,米诺环素可作为治疗该菌所致感染的首选药物,我中心各科室分离到的脑膜败血伊丽莎白菌未引起明显的暴发流行。

     

    Abstract: Objective To analyze the clinical distribution,drug resistance and homology of Elizabethkingia meningoseptica. Methods Clinical data about 44 patients infected with Elizabethkingia meningoseptica from April 2012 to December 2018 in our hospital were collected.The 44 bacteria strains were reidentified and their homology was analyzed by mass spectrometer.The drug resistance rate of Elizabethkingia meningoseptica to 12 kinds of drug was tested by broth microdilution method. Results The bacteria were mainly isolated from lower respiratory tract (accounted for 81.7%),and mainly distributed in the department of ICU,followed by department of respiratory and emergency department.The similarity of these strains was low.The resistance rates of these strains to amikacin,ciprofloxacin,sulfamethoxazole/trimethoprim and meropenem were higher than 80%,and the resistance rate of it was 100% to ceftazidime,cefepime and gentamicin.But the strains were 100% susceptible to minocycline. Conclusion Our clinical strains of Elizabethkingia meningoseptica possesses higher level of drug resistance and minocycline is the most susceptible antibiotic for therapy choice.These strains are heterogenous,and do not cause outbreak of epidemics in our hospital.
