Abstract:
Objective To explore the effect of home swallowing rehabilitation exercise developed by a multidisciplinary collaborative team on rehabilitation of patients with swallowing dysfunction after stroke.
Methods The research team collaborated with specialists from nutrition, rehabilitation, and neurology department, and developed a health education manual for home swallowing rehabilitation exercises and prevention of aspiration.A total of 167 stroke patients who were discharged from the department of neurology of our hospital were selected, and they were evaluated by the Watian water drinking test. Then 105 patients with swallowing dysfunction were screened out.Patients were randomly divided into intervention group (n=53) and control group (n=52).The control group were managed in accordance with the routine nursing discharge protocol, and the intervention group were provided with a tailored guidance for home swallowing rehabilitation and prevention of aspiration based on the results of swallowing function assessment. The intervention period was 24 weeks (from April to October in 2018).
Results The number of choking per day in the intervention group (
MdIQR,11,2) had no significant difference compared to the control group (
MdIQR,10,2)(
P>0.05);while the swallow-related muscle function score (
MdIQR,75,9
vs 53,7) and the Watian water drinking test results (
MdIQR,32,4
vs 21,3) improved significantly compared with the control group (
P<0.05,respectively).
Conclusion Based on the results of swallowing function assessment, targeted home-based swallowing rehabilitation exercises training,as well as multidisciplinary tailored guidance and intervention, are of great significance to the improvement of swallowing function.
