铁缺乏对非瓣膜病房颤患者左心房重构的影响

Influence of iron deficiency on left atrial remodeling in patients with non-valvular atrial fibrillation

  • 摘要: 目的 探讨铁缺乏对非瓣膜病房颤患者左心房重构的影响。方法 连续入选2017年12月-2019年4月于北京市回民医院心内科住院治疗的非瓣膜病房颤患者240例。检测患者铁代谢指标(血清铁、总铁结合力、铁蛋白、转铁蛋白),并计算转铁蛋白饱和度,根据铁蛋白及转铁蛋白饱和度水平,将入选患者分为铁缺乏组(135例)和非缺铁组(105例)。评价患者贫血情况,通过超声心动图测量患者左心结构,探讨铁缺乏对非瓣膜病房颤患者左心房重构的影响。结果 铁缺乏组与非缺铁组比较,NYHA心功能分级、左心房内径(4.31±0.80) cm vs (3.84±0.61) cm和左心室舒张末期内径(4.74±0.72) cm vs(4.29±0.76) cm均显著升高(P<0.05);两组患者血红蛋白水平、贫血率无统计学差异(P>0.05)。亚组分析中,无论是否伴有贫血,铁缺乏组左心房内径(4.50±1.01) cm和(4.15±0.51) cm vs (3.99±0.49) cm和(3.75±0.65) cm、左心室舒张末期内径(4.55±0.78) cm和(4.89±0.62) cm vs (4.38±1.01) cm和(4.23±0.53) cm均显著高于非缺铁组(P均<0.05)。相关性分析显示,左心房内径与血清铁(r=-0.311,P=0.001)、转铁蛋白饱和度(r=-0.362,P=0.000)、铁蛋白(r=-0.468,P=0.000)呈显著负相关。多元线性回归显示左心房内径与铁缺乏显著相关(B=-0.358,P=0.000)。结论 缺铁可能与左心房重构有一定相关性,持续进展的缺铁状态对房颤的维持具有不利影响。

     

    Abstract: Objective To investigate the influence of iron deficiency on left atrial remodeling in patients with non-valvular atrial fibrillation. Methods From December 2017 to April 2019,240 inpatients with non-valvular atrial fibrillation admitted to the department of cardiology of Beijing Moslem People's Hospital were enrolled in our study.Iron metabolism indexes (including serum iron,total iron binding capacity,ferritin and transferrin) were tested and transferrin saturation was calculated.According to the level of ferritin and transferrin saturation,135 patients were assigned into iron deficiency group and 105 cases into non-iron-deficiency group.Whether patients suffered from anemia was evaluated,and the left cardiac structure was measured by echocardiography,and the impact of iron deficiency on left atrial remodeling in patients with non-valvular atrial fibrillation was investigated. Results The level of NYHA heart function,left atrium diameter (LAD,4.31±0.80 cm vs 3.84±0.61 cm),and left ventricular end diastolic dimension (EDD,4.74±0.72 cm vs 4.29±0.76 cm) were significantly higher in iron deficiency group compared to non-iron-deficiency group (all P<0.05);however,there was no significant difference in the level of hemoglobin and the prevalence rate of anemia between the two groups (P>0.05).In subgroup analysis,whether suffered from anemia or not,LAD (4.50±1.01 cm,4.15±0.51 cm vs 3.99±0.49 cm,3.75±0.65 cm) and EDD (4.55±0.78 cm,4.89±0.62 cm vs 4.38±1.01 cm,4.23±0.53 cm) were significantly higher in atrial fibrillation patients with iron deficiency compared to atrial fibrillation patients without iron deficiency (all P<0.05).Correlation analysis showed that LAD was negatively correlated with serum iron (r=-0.311,P=0.001),transferrin saturation (r=-0.362,P=0.000),and ferritin (r=-0.468,P=0.000).Multivariate linear regression analysis showed that LAD was significantly correlated with iron deficiency (B=-0.358,P=0.000). Conclusion Iron deficiency may be a contributing factor to left atrial remodeling,and continuous progression of iron deficiency has an adverse effect on the maintenance of atrial fibrillation.
