Abstract:
Objective To evaluate a new gastric cancer screening scoring system in the screening of gastric intestinal metaplasia in the physical examination population,and explore the related risk factors of intestinal metaplasia in combination with lifestyle survey.
Methods One hundred and ninety-five cases who underwent health-check in Chinese PLA General Hospital from May 2016 to May 2018 were examined by gastroscopy.According to the gastroscopy and pathological results,they were divided into normal group and intestinal metaplasia group.Serum pepsinogenⅠ (PGⅠ),pepsinogenⅡ (PGⅡ),gastrin-17 (G-17) and Helicobactor pylori (Hp) were tested,and pepsinogen ratios (PGR) were calculated.All enrolled subjects had completed the lifestyle questionnaire.The diagnostic value of the new gastric cancer screening scoring system for intestinal metaplasia was analyzed,and the related influence factors were analyzed by multivariate logistic regression model.
Results Of the 195 cases,33 cases were in the intestinal metaplasia group and 162 cases were in the normal group.Subjects in the intestinal metaplasia group were older than those in the control group,had lower PGI and PGR,and higher G-17 (all
P﹤0.05).According to the new gastric cancer screening scoring system,the median score of the intestinal metaplasia group was 13.0 (IQR:12.0,15.5),which was higher than that of the control group (
Md:9.0;IQR:7.0,13.0) (
P=00.00).Multivariate analysis revealed that spicy diet (
OR:3.331,95%
CI:1.293-8.584) and smoking (
OR:3.490,95%
CI:1.491-8.169) were risk factors for intestinal metaplasia,and drinking coffee (
OR:0.312,95%
CI:0.120-0.810) was a protective factor.
Conclusion The new gastric cancer screening scoring system has certain value in the screening of intestinal metaplasia in the physical examination population.Spicy diet and smoking are the risk factors of intestinal metaplasia,and drinking coffee was a protective factor,but these results need to be further verified by large-scale study.
