胎儿完全型肺静脉异位引流的超声特征分析

Ultrasonographic features of fetal total anomalous pulmonary venous drainage

  • 摘要: 目的 探讨胎儿完全型肺静脉异位引流(total anomalous pulmonary venous drainage,TAPVD)的超声图像特征。方法 选取2009年6月-2019年12月在我院产前超声检查并经引产后尸体解剖或出生后超声心动图、外科手术证实的58例TAPVD作为研究对象,总结TAPVD产前超声表现及诊断思路,分析产前超声漏诊原因。结果 58例TAPVD中,心上型16例,心下型15例,心内型18例,混合型9例。58例中合并其他畸形39例(39/58,67.2%),孤立性19例(19/58,32.8%)。TAPVD合并其他畸形者产前超声全部正确诊断,孤立性TAPVD产前超声漏诊8例,漏诊率13.8%(8/58)。TAPVD产前超声表现:左心房后方无肺静脉开口,左心房与降主动脉之间距离增大;左心房后方或侧面可见共同肺静脉干,追踪共同肺静脉干及垂直静脉走行可显示引流部位,相应引流部位血管扩张;肺静脉梗阻部位彩色多普勒表现为高速湍流。结论 TAPVD具有特征性产前超声表现,产前超声可对TAPVD进行准确分型并评估是否合并肺静脉梗阻及其他畸形,孤立性TAPVD产前超声容易漏诊。

     

    Abstract: Objective To investigate the features of ultrasound imaging of fetal total anomalous pulmonary venous drainage (TAPVD). Methods A total of 58 cases diagnosed as TAPVD in our hospital from June 2009 to December 2019 were analyzed.Prenatal diagnosis was confirmed by postpartum autopsy,postpartum echocardiography or surgery.The prenatal ultrasound findings and ideas of prenatal diagnosis of TAPVD were summarized.The causes of missed diagnosis were analyzed. Results Among the 58 cases,there were 16 cases of supracardiac type,15 cases of infracardiac type,18 cases of intracardiac type and 9 cases of mixed type.We found that 39 cases (39/58,67.2%) had other malformations,and 19 cases (19/58,32.8%) were isolated TAPVD.All the cases of TAPVD complicated other malformations were correctly diagnosed by prenatal ultrasound,but diagnosis of isolated TAPVD was missed by prenatal ultrasound in 8 cases,with a missed diagnosis rate of 13.8% (8/58).The common ultrasonographic findings of TAPVD included absent insertions of pulmonary veins in the left atrium;increased distance between left atrium and descending aorta;common pulmonary vein trunk behind or on the side of left atrium,the drainage path could be detected by following the tracks of common pulmonary vein and vertical veins with vasodilation at the corresponding drainage site;pulmonary vein obstruction showing high velocity turbulence on color doppler flow imaging. Conclusion TAPVD has typical characteristics on images of prenatal ultrasound.Prenatal ultrasound can accurately classify TAPVD and assess whether it is associated with pulmonary venous obstruction and other malformations.Isolated TAPVD is easily missed diagnosed by prenatal ultrasound.
