误诊为风湿病的阿德福韦酯相关低磷性骨软化症21例分析

Adefovir dipivoxil-induced hypophosphatemia osteomalacia misdiagnosed as rheumatism:An analysis of 21 cases

  • 摘要: 目的 分析阿德福韦酯导致低磷性骨软化症的临床特点,为该病的临床诊断提供依据。方法 回顾性分析2012年1月-2019年1月解放军总医院第一医学中心诊断为阿德福韦酯导致低磷性骨软化症的21例患者,总结其临床特征。结果 21例患者平均年龄(53.86±9.98)岁,男女比例15∶6,平均服用阿德福韦酯(7.33±3.75)年(2~15年)出现骨痛症状;从出现症状到确定诊断时间为18(12~30)个月,患者均表现为多关节疼痛或骨痛,受累部位为踝(95.2%)、膝(81.0%)、肋(71.4%)、腰骶(66.7%)、髋(66.7%)等;90.5%的患者出现下肢无力,81.0%的患者出现行走困难,可误诊为强直性脊柱炎、骨质疏松症等。所有患者均伴有低磷血症,12例(57.1%)符合范可尼综合征的诊断。全身骨扫描动态显像均显示全身多发性放射浓聚。结论 长期服用阿德福韦酯可导致低磷性骨软化症,表现为骨痛、关节痛、骨质疏松等,易误诊为风湿病,因此使用阿德福韦的患者应定期行电解质检测及影像学检查,避免误诊误治。

     

    Abstract: Objective To analyze the clinical features of hypophosphatemia osteomalacia induced by adefovir dipivoxil and provide evidence for evaluation of the condition. Methods A retrospective analysis of 21 patients diagnosed as adefovir dipivoxil-induced osteomalacia from January 2012 to January 2019 in the First Medical Center of Chinese PLA General Hospital was conducted to summarize their clinical characteristics. Results The average age of the 21 patients was 53.86±9.98 years,with male to female ratio of 15:6.The time from taking adefovir dipivoxil to onset of bone pain was 2-15 years,with an average of 7.33±3.75 years.The time from symptom onset to diagnosis was 18 (12-30) months.Most patients complained of polyarticular pain or bone pain,and the affected areas included ankles (95.2%),knees (81.0%),rib (71.4%),lumbosacral (66.7%),hip (66.7%),etc.Lower limb weakness developed in 90.5% of patients,and 81.0% of patients had walking difficulty.The conditions could be misdiagnosed as ankylosing spondylitis,osteoporosis,etc.Hypophosphatemia was found in all cases,and 12 cases (57.1%) were in accordance with the diagnosis of Fanconi syndrome.The bone scan showed multiple increased radiopacities in the whole body. Conclusion Long-term use of adefovir dipivoxil may cause hypophosphatemia osteomalacia,most of which manifests with bone pain,arthralgia and osteoporosis.Patients taking adefovir should undergo electrolyte testing and imaging examination regularly to avoid misdiagnosis and mistreatment.
