新冠疫情对解放军总医院第一医学中心异地医保住院患者收容的影响

Influence of COVID-19 on hospitalization volume of patients with off-site medical insurance

  • 摘要: 目的 分析新冠疫情对解放军总医院第一医学中心异地医保患者收容的影响。方法 回顾性分析解放军总医院第一医学中心2020年2月1日-4月30日及2019年同期异地医保全院及各科收容情况。结果 疫情期间异地医保住院收容人次整体下降73.81%;住院次均费用(36 019元 vs 46 186元,P=0.00)和平均住院时间(8.5 d vs 11.8 d,P=0.00)均明显增长。与2019年同期相比,收容前10位科室基本相同,其中内科科室30%,外科科室70%;收容人次前10位科室占全院异地医保收容比例均较大,2019年占比66.48%,2020年占比72.81%;其中,肿瘤内科是收容人次最多的科室。疫情对医院整体收容人次影响显著,收容人次下降80%及以上的科室15个,下降60%~80%的科室有17个,下降40%~60%的科室有5个。疫情前后异地医保患者的主要来源地域基本相同。结论 新冠肺炎对异地医保住院收容人次影响显著,整体下降74%。

     

    Abstract: Objective To investigate the influence of coronavirus disease 2019 (COVID-19) on the hospitalization volume of patients with off-site medical insurance in a grade A tertiary hospital in a first-tier city. Methods A retrospective analysis was performed for the hospitalization volume of patients with off-site medical insurance in all departments of a grade A tertiary hospital in a first-tier city from February 1,2020 to April 30,2020 and during the same period of 2019. Results During the epidemic of COVID-19,the overall hospitalization volume of patients with off-site medical insurance decreased by 73.81%,and there were significant increases in average cost (36 019 yuan vs 46 186 yuan,P=0.00) and average length of stay (8.5 d vs 11.8 d,P=0.00) for each hospitalization.The top 10 departments with high hospitalization volume were basically the same as those during the same period last year,among which departments of internal medicine accounted for 30% and surgical departments accounted for 70%.These top 10 departments accounted for a large proportion of the patients with off-site medical insurance who were admitted to the hospital,with 66.48% in 2019 and 72.81% in 2020,among which department of medical oncology had the highest number of hospitalized patients.The epidemic had great impact on the overall hospitalization in the hospital,and there were 15 departments with a reduction of 80% or more in hospitalization volume,17 with a reduction of 60%-80%,and 5 with a reduction of 40%-60%.There was no significant change in the regions where the patients with off-site medical insurance came from epidemic. Conclusion COVID-19 has great impact on the hospitalization of patients with off-site medical insurance,and the overall hospitalization volume decreases by 74%.
