功能性腹壁封闭敷料的制备及其性能测试

Development and performance testing of a novel functional abdominal dressing

      背景  腹部开放性创伤是一种常见战创伤,尽快封闭腹腔并保护腹腔脏器对提高伤病员救治率具有重要意义。
      目的  研发一种新型腹壁闭合敷料,使其能够满足腹壁开放性损伤救治过程中快速止血、封闭缺损和实时反映腹腔压力的需求。
      方法  通过静电纺丝的方法制备热塑性聚氨酯弹性体(thermoplastic polyurethane,TPU)静电纺丝纳米纤维膜并组装TPU纳米纤维膜电容式压力传感器;使用生物黏合剂将电容式压力传感器与壳聚糖止血敷料组装为功能性腹壁封闭敷料;对该敷料的体外压力传感性能、力学性能、生物相容性和止血性能进行检测;通过构建动物模型对其在体内的压力传感性能进行检测。
      结果  该腹壁封闭敷料弹性高于普通医用敷料;在体外测试中,其压力传感性能优异,生物相容性、止血性能良好;在腹部开放性创伤模型中,可以对动物腹内压进行实时监测。
      结论  本研究通过将电容式压力传感器内嵌于壳聚糖止血敷料中,实现了对腹内压的实时监测,有望用于开放性腹部创伤的紧急救治。

     

    Abstract:
      Background  Penetrating abdominal trauma is common in war, and in order to improve cure rate, it is necessary to close the wound as soon as possible to protect the abdominal organs.
      Objective  To develop a novel functional abdominal wall sealing dressing, which can meet the need for rapid hemostasis, defect closure and real-time monitoring of abdominal pressure in the emergency treatment of open abdominal wall injury.
      Methods  Thermoplastic polyurethane (TPU) nanofiber membrane was prepared by electrospinning and a TPU capacitive pressure sensor was assembled. Then, the TPU capacitive pressure sensor was assembled with chitosan hemostatic dressing by biological adhesive to prepare a functional abdominal wall sealing dressing. The dressing was tested in terms of in vitro pressure sensing performance, mechanical property, biocompatibility and hemostatic performance, and an animal model was established to test in vivo pressure sensing performance.
      Results  The elasticity of the dressing was better than that of the conventional medical dressing. In the in vitro test, it showed excellent pressure sensing performance, biocompatibility and hemostatic performance. In an open abdominal wall injury model, the intra-abdominal pressure could be monitored in real-time by the functional dressing.
      Conclusion  In this study, a capacitive pressure sensor is embedded into chitosan hemostatic dressing to enable the real-time monitoring of intra-abdominal pressure, and therefore, it is expected to be used in the emergency treatment of penetrating abdominal injury.
