健康老年人群尿液NAG及NAG/Crea参考范围建立的探讨

Establishing urinary NAG and NAG/Crea reference range in healthy elderly population

      背景  尿液N-乙酰-β-氨基葡萄糖酶(N-acetyl- β- glucosaminase,NAG)/肌酐(creatinine,Crea)是早期肾损伤的一个敏感指标,近年来备受关注。但目前没有该指标在老年群体中的参考范围。
      目的  探讨健康老年人群尿液NAG和NAG/Crea参考范围。
      方法  收集2021年4 - 7月在解放军总医院第二医学中心915例健康查体者的尿液标本,将标本按照年龄<60岁、≥60岁分为青壮年组和老年组,对两组尿液标本进行NAG、NAG/Crea检测,比较检测指标差异。
      结果  青壮年组尿液NAG、NAG/Crea中位值分别为9.93(7.18,13.7) U/L、8.33(6.49,11.09) U/g·Cr,老年组尿液NAG、NAG/Crea中位值分别为8.93(6.58,12.53) U/L、10.19(7.76,14.05) U/g·Cr。尿液NAG参考范围的制定无需区分性别、年龄(Z均<Z*),为3.53 ~ 24.48 U/L;尿液NAG/Crea参考范围的制定需区分年龄(ZZ*),<60岁人群为4.24 ~ 21.93 U/g·Cr,≥60岁老年人群为4.61 ~ 27.83 U/g·Cr。
      结论  初步建立了老年人群尿NAG、NAG/Crea的参考范围,为临床医生判断早期肾损伤提供参考。

     

    Abstract:
      Background  Urine N-acetyl- β- Glucosaminase/creatinine (NAG/Crea) is a sensitive index of early renal injury, which has attracted much attention in recent years. However, there is no reference range for the elderly group.
      Objective  To explore the reference range of urine NAG and NAG/Crea in healthy elderly population.
      Methods  Urine samples from 915 cases of health examination in the Second Medical Center of Chinese PLA General Hospital from April to July in 2021 were collected. The samples were divided into young group and old group according to age (cut-off: 60 years). The urine samples of the two groups were tested for NAG and NAG/Crea, and the differences between the two groups were compared.
      Results  The urinary NAG and NAG/Crea in the young group were 9.93 (7.18, 13.7) U/L and 8.33 (6.49, 11.09) U/g·Cr, and they were 8.93 (6.58, 12.53) U/L and 10.19 (7.76, 14.05) U/g·Cr in the old group. The reference range of urine NAG did not need to distinguish gender and age (Z<Z*), which was 3.53 - 24.48 U/L. However, the formulation of urine NAG/Crea reference range needed to distinguish the age (Z>Z*), it was 4.24 - 21.93 U/g·Cr for under 60 years old group, and 4.61 - 27.83 U/g·Cr for the more than 60 years group.
      Conclusion   The reference range of urinary NAG and NAG/Crea in the elderly population is preliminarily established in this study, which provides a reference for clinicians to judge early renal injury.
