刚果(金)南基武省维和人员疾病流行病学特征分析

Epidemiological characteristics of diseases for peacekeeper in South KIVU of Democratic Republic of Congo

      背景  中国二级医院是刚果(金)南基武省维和行动中医疗保障的关键力量,在全球新型冠状病毒肺炎(COVID-19)暴发流行的背景下维和人员疾病特点是医疗保障的重要资料。
      目的  探讨刚果(金)南基武省维和人员的疾病流行病学特征,为任务区医疗保障提供参考。
      方法  收集2019年9月- 2020年9月进入刚果(金)南基武省中国二级医院的各国所有维和人员的病历资料,分析疾病在病种、国家、月份的分布情况。
      结果  于中国二级医院就诊的维和伤病员共计951人次,门诊880(880/951,92.53%)人次,住院71(71/951,7.47%)人次。门诊伤病员主要的疾病种类:创伤疾病195人次(195/880,23.41%),口腔疾病192人次(192/880,21.82%)。住院伤病员主要疾病种类:消化系统疾病18人次(18/71,25.35%),创伤疾病11人次(11/71,15.49%)。门诊与住院伤病员中传染性疾病共44人次(44/951,4.63%)。就诊人次较多的国家:巴基斯坦345人次,埃及92人次,中国91人次,尼泊尔90人次,乌拉圭80人次。疾病在月份分布上呈现先逐步上升,于2020年1月达到最高(19.84%)后开始逐步下降的趋势;2020年5月首次出现疑似和确诊COVID-19病例,并持续出现。
      结论  刚果(金)南基武省维和人员疾病类型主要为创伤与口腔疾病,伤病员主要出现在巴基斯坦、埃及、中国等出兵人数多的国家。受COVID-19的影响,2020年2月以后疾病例数呈现下降趋势,但就诊病例多为具有感染性的急危重症,需要严密防护和妥善处置。

     

    Abstract:
      Background  Chinese Level Ⅱ Hospital is a key force of medical security in the peacekeeping operation in South KIVU of Democratic Republic of Congo. In the context of the global COVID-19 outbreak, the disease characteristics of peacekeepers are important information for medical security.
      Objective  To investigate the epidemiological disease characteristics of peacekeeper so as to provide reference for medical operation in the Democratic Republic of Congo.
      Methods  The epidemiological data of the peacekeeper, including the gender, age, nationality and the epidemiologic feature of disease in South KIVU from September 2019 to September 2020 were recorded. The distribution of diseases in the population, country and time were analyzed.
      Results  During the task period, a total of 951 patients admitted to Chinese Level Ⅱ Hospital which included 880 outpatients (92.53%) and 71 inpatients (7.47%). The main diseases of outpatients were trauma disease (195/880, 23.41%) and oral diseases (192/880, 21.82%). The main diseases of inpatients were digestive diseases (18/71, 25.35%) and traumatic diseases (11/71, 15.49%). A total of 44 infectious diseases were found in outpatients and inpatients (44/951, 4.63%). The patients were mainly from Pakistan (345), Egypt (92), China (91), Nepal (90) and Uruguay (80). The number of visits began to decline after reaching the peak (19.84%) in January 2020. The first breakout of suspected and confirmed COVID-19 cases was found in May 2020, and appeared for the following months.
      Conclusion  The main types of diseases in Congo are traumatic diseases and oral diseases, and the wounded and sick mainly appear in Pakistan, Egypt, China and other countries with a large number of troops. After February 2020, the number of visits shows a decline affected by COVID-19. However, most patients are infectious critical diseases, so rigorous protection and proper disposal are required.
