剥脱性激光治疗面部萎缩性痤疮瘢痕的疗效与影响因素分析

Clinical outcomes and associated factors of ablative lasers in treatment of atrophic acne scar

      背景  不同剥脱性激光治疗萎缩性痤疮瘢痕的疗效及其影响因素临床资料有限。
      目的  探讨不同剥脱性激光治疗面部萎缩性痤疮瘢痕的临床疗效和影响因素。
      方法  回顾性分析2014年7月 - 2022年8月在解放军医学院第七医学中心皮肤科激光中心采用剥脱性激光治疗面部萎缩性痤疮瘢痕患者的临床资料,包括二氧化碳点阵激光(Deep模式,能量密度12.5 ~ 22.5 mJ,光斑密度5% ~ 10%)、铒像素激光(能量800 ~ 1 400 mJ,能量密度3 ~ 6 J/cm2)及等离子体射频(滚动式功率60 ~ 70 W,3 ~ 4遍/次),间隔1 ~ 12个月治疗2次。对患者治疗前后照片采用痤疮瘢痕评估量表(Echelle d’Evaluation Clinique des Cicatrices d’acne,ECCA)评分以评估疗效,并根据ECCA评分下降率,分为有效组和无效组,分析影响治疗疗效的影响因素。
      结果  共纳入61例患者,治疗前ECCA评分为(110.16 ± 30.54)分,2次治疗后为(85.98 ± 25.03)分(P<0.001),临床有效率为31.15%。Logistic回归提示治疗前ECCA评分是治疗效果的独立影响因素(OR=0.297,95% CI:0.090 ~ 0.976,P=0.046),而年龄对治疗效果无显著影响。
      结论  剥脱性激光治疗面部萎缩性痤疮瘢痕有较好疗效,治疗前ECCA评分是治疗效果的独立影响因素。

     

    Abstract:
      Background  There are limited data about the effect of different types of ablative lasers for the treatment of atrophic acne scars and its related clinical factors.
      Objective  To evaluate the effect of different types of ablative lasers in the treatment of facial atrophic acne scars and its related clinical factors.
      Methods  Clinical data about patients with atrophic acne scars who were treated with ablative lasers in the Laser Center of Dermatology Department in the Seventh Medical Center, Chinese PLA General Hospital from July 2014 to August 2022 were retrospectively analyzed, using fractional CO2 laser (deep mode with 12.5-22.5 mJ energy density and 5-10% spot density), Er: YAG pixel laser (energy 800-1400 mJ and fluence 3-6 J/cm2) or plasma radiofrequency (three or four passes of the roller tip at 60 to 70 W) for two sessions with 1-12 months period. Photos of patients before and after two sessions of treatments were evaluated with the Echelle d’ Evaluation Clinique des Cicatrices d’ acne (ECCA) to determine the efficacy. According to the decline level of ECCA scores, patients were divided into effective group and non-effective group to ascertain the related clinical factors affecting efficacy.
      Results  Totally 61 patients (35 males and 26 females) were enrolled. ECCA scores declined from (110.16 ± 30.54) to (85.98 ± 25.03) after two sessions of treatments (P=0.000), and the clinical effective rate was 31.15%. Multivariate logistic analysis suggested that degree of scar severity was an independent risk factor for clinical outcome (OR=0.297, 95% CI: 0.090-0.976, P=0.046), while age had no significant effect on efficacy.
      Conclusion  Ablative lasers are effective in the treatment of atrophic acne scars, degree of scar severity is independent clinical factors affecting its efficacy.
