Abstract:
Background The medical record front page plays a crucial role in medical quality control, hospital management, scientific research, and medical insurance payment, etc. The medical record front page of military hospitals has not been revised for many years.Objective To develop the standard for the basic data set of the first page of medical records in military hospitals, and establish the essential and most core data elements, in order to promote the standardized management of the first page of medical records in military hospitals. Methods Based on the current first page of medical records in military hospitals and the latest national version, this study constructs the required data items for the first page of medical records in military hospitals through literature analysis, policy research, comparative analysis, and expert meetings. Referring to the basic dataset and data element standards of the first page of medical records in China, and adhering to relevant standard specifications, the basic dataset for the first page of medical records in military hospitals is constructed using standardized methods. Results Based on the 130 data items currently used in the first page of military hospital medical records, 28 data items and the data dictionary were revised, 26 data items were abolished, and 108 new data items were added, completing the revision of the new first page of military hospital medical records. Based on the data items of the new first page, a basic data set and data element specifications for the first page of military hospital medical records were developed, resulting in 294 data elements and 81 value domain code tables.Conclusion The revision of the new first page of military hospital medical records and the establishment of the basic data set standards for the first page of military hospital medical records have made the data entry on the first page more standardized and objective, minimizing the subjective judgment of medical staff. This will standardize the relevant data on the first page of military hospital medical records,ensuring consistency and comparability of the information collected, stored, published, and exchanged, supporting the construction of medical information systems, promoting information sharing, and improving data utilization.
