青年心脑血管疾病患者同型半胱氨酸水平及干预研究

Homocysteine concentration in patients with ischemic stroke or coronary heart disease,and intervention study

  • 摘要: 目的: 了解保定地区青年脑梗死和冠心病患者(年龄小于45岁)血清同型半胱氨酸(homocysteine,HCY)水平及给予叶酸、维生素B12治疗后HCY的变化。方法: 对60例脑梗死患者,60例冠心病患者及60例健康人进行血清HCY的检测,各组随机分成干预组与非干预组,每组30例,干预组给予叶酸5 mg,1/d,维生素B12250μg,3/d,口服2个月后复查。结果: 干预前三组HCY水平各不相同,脑梗死组为(16.5±5.4)μmol/L,冠心病组为(14.5±4.9)μmol/L,与正常对照组(12.1±2.9)μmol/L比较,差异有显著性意义(P<0.01)。干预后,患者的血HCY水平均有下降。结论: 高HCY血症是青年脑梗死及冠心病患者的独立致病因素,补充营养元素有助于降低血HCY水平。

     

    Abstract: Objective: To investigate homocysteine(HCY) concentration in ischemic stroke patients and in coronary heart disease(CHD) patients in comparison with normal controls and to evaluate whether providing folate and vitamin B12 could decrease serum homocysteine concentration.Methods: Serum HCY levels of 60 patients with cerebral infarction(CI) and 60 patients with CHD and 60 normal controls were measured.Then each group was randomly divided into two groups in which one group was provided with folate 5mg qd,and vitamin B12 250μgtid while the other was not provided with folate and vitamin B12.These patients were reexamined after 2 months.Results: The mean serum HCY level was(16.5±5.4)μmol/L for cerebral infarction patients,(14.5±4.9) μmol/L for CHD,and(12.1±2.9) μmol/L for normal subjects.Statistically,the mean serum HCY levels of cerebral infarction and CHD patients were significantly higher than those of the normal subjects(P<0.01).The HCY concentration was lower in the groups provided with foliate and vitamin B12 than that in the groups without folate and vitamin B12(P=0.0001),even in the normal youngers.Conclusions: The HCY concentration is higher in the patients with ischemic stroke or CHD.Folate supplement could moderately lower HCY.
