VEGF和MVD在肺癌及其脑转移灶中的表达及意义

Expression and significance of microvascular density and vascular endothelial growth factor in lung cancer and their intracranial metastasis

  • 摘要: 目的:研究血管内皮生长因子及微血管密度在肺癌及其脑转移灶中的表达及意义。方法:应用免疫组化SP法检测34例肺癌、24例肺癌颅内转移患者癌组织MVD和VEGF的表达,结合预后资料观察这两个指标判断预后的价值。结果:肺癌中VEGF高表达者15例(44.1%),MVD高表达者19例(55.9%);颅内转移瘤中VEGF高表达者12例(50.0%),MVD高表达者13例(54.2%)。颅内转移瘤MVD和VEGF的表达水平显著高于原发灶。肺癌MVD及VEGF的表达与年龄、吸烟史和肿瘤大小无关,与肿瘤淋巴结转移情况及TNM分期显著相关。单因素生存分析显示MVD、VEGF与肺癌患者术后无病存活期(DFS)及总生存期(OS)有显著相关性;多元回归分析显示VEGF为独立预后因子,它的参数为正值,属于风险因子,表达越高,生存期越短。结论:VEGF和MVD可能为判断肺癌预后的有效指标,其中VEGF可能是较强的独立预后因子。

     

    Abstract: Objective: To study the expression and clinical significance of vascular endothelial growth factor (VEGF) and microvascular density (MVD) in lung cancer and their intracranial metastasis.Methods: Applying SP immunohistochemical technique, expression of MVD and VEGF were studied in 34 patients with lung cancer and 24 patients with intracranial metastasis. Results: For patients with lung cancer,There're 15 of them with high VEGF expression(44.1%),19 of them with high MVD expression(55.9%);for patients with intracranial metastasis,There're 12 of them with high VEGF expression(50%);13 of them with high MVD expression(54.2%).Both VEGF and MVD in the intracranial matastasis were significantly higher than in lung cancer.Lymph node status and TNM stage were significantly correlated with MVD and the expression of VEGF;age, smoking history and tumor size have no correlation with them.After survival analysis,we found that MVD and VEGF predicated shorter disease-free survival (DFS) and overall survival (OS);in multivariate analysis,VEGF was an independent predicators for DFS and OS,it was a risk predicator because of their positive statistic value,the higher they express,the shorter survival time for the patients.Conclusion: Both VEGF and MVD are all useful factors in predicting prognosis of patients with lung cancer.VEGF appears to be a strong and independent biologic marker for predicting prognosis of lung cancer patients.
