肌内埋置神经断端预防残端神经瘤的组织学与电生理学的实验研究

Experimental Histological and Electrophysiological Observations on Implantation of Neural Stump into Muscle

  • 摘要: 本实验通过组织学及电生理学观察神经近断端埋入邻近肌肉来预防治疗残端神经瘤。实验选用SD大白鼠16只,采用双侧坐骨神经自身对照。结果表明:未处理的神经断端在术后1个月即有典型残端神经瘤形成,神经瘤内可见成肌纤维细胞;而神经近断端经肌肉埋入后,神经断端纤维分散长入肌纤维间,其组织学变化及电生理学变化与典型神经瘤相异,无成肌纤维细胞。因此,肌内埋置神经近断端可预防和治疗残端神经瘤引起的疼痛。

     

    Abstract: Prevention of traumatic neuroma by implantation of neural stump into muscle was studied in SD rats. Electrophysiological, histological and TEM were used to assess the effects of implantation, and it was compared with the conventional treatment of neural stump, at 1,2,3, and 6 months postoperatively. The results show that neuroma forms and there are myofibroblasts in the neural stumps of conventional treatment. But in cases of implantation the nerve fibers dispersed into muscle fibers, and there are no myofibroblasts found. The causes of intractable pain,prevention and treatment of the painful neuroma, and the mechanisms of our method are discussed.
