肿瘤患者下呼吸道感染病原学及耐药分析

Analysis of etiology and drug resistance in patients with lower respiratory tract infection and tumor

  • 摘要: 目的 探讨我院肿瘤科恶性肿瘤继发下呼吸道感染病原菌的分布及耐药性。方法 采用回顾性调查方法,对244例恶性肿瘤下呼吸道感染标本进行病原菌分布及耐药性分析。结果 下呼吸道感染菌主要为革兰阴性杆菌,其中肺炎克雷伯菌肺炎亚种分离率最高(24.01%),铜绿假单胞菌第二(20.96%),大肠埃希菌第三(20.52%)。结论 我科肿瘤患者下呼吸道感染细菌构成比与其他医院有一定差别。第三代头孢、氨基糖甙类药物的耐药率低于其他综合性医院,可作为我科患者继发感染时的经验性一线用药。

     

    Abstract: Objective To evaluate the distributions and drug resistance of the pathogenic bacteria causing lower respiratory tract infection in patients with malignant tumor. Methods A retrospective study was conducted of the distributions and drug resistance of the pathogenic bacteria in the lower respiratory tract specimens of 244 patients with malignant tumor and concurrent lower respiratory tract infection. Results The most common pathogens causing lower respiratory tract infection are Gram-negative bacilli,24.01% of which are K.pneumoniae,20.96% P.aeruginosa,and 20.52% E.coli. Conclusion The bacterial constitution in the lower respiratory tracts of patients with tumor in our tumor ward is different from that in other hospitals. The drug-resistance rate of the third generation cephalosporin and amino glycoside antibiotics in our hospital is lower than that in other hospitals. Therefore,they should be used as experiential first-line drugs.
