老年大鼠呼吸机相关肺损伤药物治疗的研究

Study of pharmacotherapy in ventilator-induced lung injury in an old rat model

  • 摘要: 目的: 研究沙丁胺醇对老龄大鼠呼吸机相关肺损伤的防护作用。方法: 24只健康雄性wista老龄大鼠随机分为3组,即正常对照组,大潮气量致伤组和沙丁胺醇防护组。通气4h后处死,分别测定肺湿干比(W/D)、支气管肺泡灌洗液(BALF)总蛋白含量、肺匀浆总一氧化氮合酶(tNOS)与诱导型一氧化氮合酶(iNOS)活性,并观察肺病理改变。结果: B组W/D值、总蛋白含量较A、C组显著升高(P<0.01)。B组tNOS、iNOS活性显著降低,C组较B组显著提高。病理显示B组较A、C组肺内有更多的白细胞浸润。结论: 在VILI老龄大鼠tNOS、iNOS活性降低,沙丁胺醇通过提高tNOS、iNOS活性对老龄大鼠VILI模型有较明显的防护作用。

     

    Abstract: Objective: To investigate the protective effects of salbutamol on ventilator-induced lung injury in an old rat model. Methods: 24 healthy male wista rats were randomly divided into three groups.Group A served as control group,Group B received large tidal volume ventilation,and Group C received the same tidal volume ventilation as Group B and was treated with salbutamol.After ventilation,all the rats were killed.We detected wet to dry right lung weight ratio(W/D),the concentration of total protein and albuminin in bronchoalveolar lavage fluid(BALF)and the activity of tNOS and iNOS in lung tissue bomogenate.Lung histopathology of the three groups was assessed. Results: Wet to dry right lung weight ratio and the concentration of total protein and albuminin in BALF were significantly higher in Group B than in Group A and Group C.The activity of tNOS and iNOS in lung tissue bomogenate was significantly lower in Group B than in Group A and Group C.Histopathologic findings demonstrated more infiltrating WBC in Group B than in Group A and Group C. Conclusion: The results suggest that the activity of tNOS and iNOS decreases in lungs of VILI old rats and salbutamol has obvious protective effects on VILI by increasing the activity of tNOS and iNOS.
