定量降钙素原测定在血流感染诊断中的应用

Application of quantitative procalcitonin in diagnosis of blood stream infection

  • 摘要: 目的 结合血培养,评价血清降钙素原定量检测对血流感染的诊断作用。方法 对我院2009年6月-2010年4月同时进行血培养与降钙素原检测患者资料进行回顾性分析,应用全自动连续性检测系统BacT/Alert3D进行血培养,VITEK II微生物全自动鉴定仪对病原菌进行鉴定,应用法国梅里埃mini-VIDAS全自动免疫荧光分析仪(B.R.A.H.M.S PCT assay)检测降钙素原(PCT)。应用SPSS17.0统计软件进行统计学分析。结果 506份同时送检的标本,PCT和血培养的阳性率分别为43.9%和20.6%;以0.70为截点,PCT诊断的敏感性为87.5%、特异性为67.4%,阳性预测值和阴性预测值分别为41.0%、95.4%。结论 定量PCT检测操作简单,对于血流感染诊断有一定的敏感性和特异性,可连续监测患者感染状况,能及时为临床决策提供依据。

     

    Abstract: Objective To evaluate the role of combined quantitative procalcitonin(PCT) and blood culture in diagnosis of blood stream infection. Methods PCT and blood culture data about patients admitted to our hospital from June 2009 to April 2010 were retrospectively analyzed.Blood samples were cultured with the automatic BacT/Alert 3D system.Pathogenic bacteria were identified using the VITEK II system.PCT was assayed using the mini-VIDAS.B.RA..H.MS PCT.Data were analyzed using the SPSS17.0 statistical software. Results The positive rate of PCT and blood culture was 43.9% and 20.6%,respectively.The sensitivity and specificity of PCT were 87.5% and 67.4%,respectively,when 0.70 was taken as a cut-off value.The positive and negative predictive value for PCT was 41.0% and 95.4%,respectively. Conclusion Quantitative PCT can be used in diagnosis of blood stream infection with a rather good sensitivity and specificity by continuously monitoring infection in patients,and provide evidence for clinical decision-making.
