Aspiration pneumonia in mechanical ventilation: analysis of 31 patients

  • 摘要: 目的:提高对机械通气时吸入性肺炎的认识。方法:分析31例机械通气患者并发吸入性肺炎的临床特征。结果:31例患者共发生59例次吸入性肺炎,其发生率与人工气道的时间呈正相关,大多数发生于气管插管患者(43例次,72.9%)。常见诱因包括鼻饲方式及吸痰方法不当,人工气道时间长及留置胃管等。临床表现为发热、咯痰、呼吸困难、肺内湿 音、外周血白细胞升高以及胸片发现新的浸润性阴影等。病原微生物以革兰阴性菌为主,绿脓杆菌和克雷白杆菌位居前二位。17例患者拔管脱机,14例死亡。结论:吸入性肺炎是机械通气的常见并发症之一,积极预防和治疗能改善其预后。

     

    Abstract: Objective: To enhance our awareness of aspiration pneumonia following mechanical ventilation.Methods: 31 cases of mechanically ventilated patients with aspiration pneumonia were reviewed.Results: There were 59 case times of aspiration pneumonia diagnosed in these patients. The incidence was directly proportional to the duration of artificial airway. Most of aspiration pneumonia appeared in endotracheal intubation patients (43 cases, 72.9%). The high risk factor included wrong manner of feeding through nasogastric tube and aspiration of sputum, long lasting time of artificial airway and feeding tube placing. The clinical manifestations were fever, productive cough, dyspnea, pulmonary moist rales, peripheral neutrophilia and new infiltration patches on chest radiograph. Gram negative bacteria (especially Pseudomonas aeruginosa and klebsiella pneumonia) were the dominant aerobic pathogens. 17 cases had successful extubation but the other 14 patients died.Conclusion: Aspiration pneumonia is the common complication of mechanical ventilation. The prognosis may be improved by aggressive treatment and reasonable prophylaxis.
