血管内皮生长因子在早期脊柱关节病中的变化

Expression of vascular endothelial growth factor (VEGF) in early spondyloarthropathy patients

  • 摘要: 目的: 探讨血管内皮生长因子(VEGF)在早期脊柱关节病(SpA)外周关节病变发病机制中的作用以及与疾病活动性的相关性。方法: 采用免疫组织化学及原位杂交方法检测SpA患者及类风湿关节炎(RA)外周膝关节滑膜组织VEGF mRNA及蛋白的表达,并将VEGF检测结果进一步与关节镜下滑膜形态学所见及部分实验室炎性指标进行相关性分析。结果: SpA组滑膜衬里层VEGF蛋白的表达与CRP呈正相关,早期RA滑膜血管周围VEGF蛋白的表达与CRP呈正相关;早期RA滑膜衬里层和衬里下层基质组织VEGF蛋白表达与外周血小板计数(PLT)呈正相关。SpA组滑膜衬里层VEGF蛋白表达与血管形态、直行血管走行方式呈正相关,SpA组血管周围VEGF蛋白表达与滑膜增生程度呈正相关;RA滑膜衬里层VEGF蛋白表达与血管形态、弯曲血管走行方式、血管密度呈正相关。结论: SpA滑膜组及RA滑膜组VEGFmRNA以及蛋白表达不同主要表现在血管周围,推测这可能是造成SpA与RA预后不同的原因之一。

     

    Abstract: Objective: To explore the expression of vascular endothelial growth factor(VEGF) in synovium and understand its correlation with the pathogenesis of early spondyloarthropathy(SpA). Methods: VEGF mRNA and protein were detected by means of in situ hybridization and immunohistochemistry.Synovial levels of VEGF protein expression was evaluated to make sure whether it correlated with disease activity and macroscopic observation during arthroscopy. Results: Significant correlations were found between VEGF expression in synovium lining layer region and CRP in early SpA,between VEGF expression in perivascular region and CRP in early RA,and between VEGF expression in synovium lining layer region and sub-lining region and PLT in early RA.The expression of VEGF demonstrated its correlation with vascular morphology(VM) and straight vessels(SV) in synovium lining layer in early SpA synovial membrane.The correlation was found between VEGF expression and synovial hypertrophy in perivascular region in early SpA synovial membrane.Other correlations were found between VEGF expression and vascular morphology,tortuous vessels and vascular density in early RA synovium lining layer region. Conclusion: The high correlation between VEGF expression in perivascular region and synovial hypertrophy in SpA may be potentially useful new markers of SpA disease activities.
