人脐带间充质干细胞治疗失代偿期肝硬化的疗效观察

Therapeutic effect of human umbilical cord mesenchymal stem cell transplantation on decompensated cirrhosis

  • 摘要: 目的 观察人脐带间充质干细胞(human umbilical cord mesenchymal stem cells,HUC-MSCs)治疗失代偿期肝硬化患者的疗效。 方法 24例肝硬化失代偿期患者,经HUC-MSCs移植,于移植前、移植后1、2、4、8、12周检测谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBiL)、凝血酶原时间(PT)和白蛋白(ALB)水平。于移植前、移植后8、12周分别行CT检查,观察临床症状改善情况及不良反应。 结果 移植后2周各项肝功能指标无显著变化(P>0.05);移植后4周,ALT由(106±18) IU/L降至(82±12) IU/L,AST由(121±23) IU/L降至(87±14) IU/L,较移植前明显改善(P<0.05),其余指标无明显改善;移植后8周各项肝功能指标均有改善(P<0.05)。12周有显著改善(P<0.01)。临床症状随时间进展逐步改善且无不良反应。肝脏最大横截面积改善(P<0.05),但肝脏CT值无显著变化(P>0.05)。 结论 HUC-MSCs移植可显著改善失代偿期肝硬化患者肝功能及临床症状,并能促进肝脏修复。

     

    Abstract: Objective To observe the therapeutic effect of human umbilical cord mesenchymal stem cell(HUC-MSC) transplantation on patients with decompensated cirrhosis. Methods ALT,AST,TBiL,PT,ALB levels were measured in 24 patients with decompensated cirrhosis before and 1,2,4,8,12 weeks after they underwent HUC-MSC transplantation.Their clinical symptoms and adverse reactions were observed by CT before and 8,12 weeks after HUC-MSC transplantation. Results No significant improvement was found in liver function of the patients 2 weeks after HUC-MSC transplantation(P>0.05).The ALT and AST levels were significantly lower 4 weeks after HUC-MSC transplantation than before HUC-MSC transplantation((82±12)IU/L vs(106±18)IU/L,(87±14)IU/L vs(121±23)IU/L,P<0.05).No significant improvement was observed in the other liver function indications of the patients.All the indications of liver function obviously improved 8 and 12 weeks after HUC-MSC transplantation(P<0.05).The clinical symptoms of the patients alleviated with the prolongation of time.No adverse reaction occurred.The liver maximum cross-sectional area was significantly larger after HUC-MSC transplantation(P<0.05) with no significant change in CT value of liver(P>0.05). Conclusion HUC-MSC transplantation can significantly improve the liver function and clinical symptoms of patients with decompensated cirrhosis and promote their liver repair.
