联合不同术式治疗重度阻塞性呼吸睡眠暂停综合征
Treatment of obstructive sleep apnea syndrome by combined genioglossus advancement or hyoid suspension with uvulopalatopharyngoplasty

摘要: 目的: 联合应用悬雍垂腭咽成形术、颏舌肌前徙术和舌骨悬吊术治疗阻塞性呼吸睡眠暂停综合征。方法: 在26例患者结合不同体位的X线头影测量侧位片观察和测量以及多导睡眠仪检查结果的基础上,应用悬雍垂腭咽成形术、颏舌肌前徙术和舌骨悬吊术等不同手术方式组合。术后随访包括术区检查及与术前同样的检查。结果: 应用不同手术方式结合治疗可以明显地扩大上、下咽腔的气道,症状改善非常明显。结论: 由于阻塞性呼吸睡眠暂停综合征患者的阻塞部位和阻塞程度各不相同,有针对性的选择手术方式进行治疗是合理的,也是有效的。Abstract: Objective:This study evaluated the effectiveness of uvulopalatopharygoplasty（UPPP） with genioglossus advancement and hyoid suspension for the treatment of patients with obstructive sleep apnea syndrome（OSAS） in light of their degrees and obstructive sites.Methods:Combined UPPP and genioglossus advancement and hyoid suspension were performed in 26 patients after observing the cephalometric radiographs and analyzing the PSG signals.After operation the cephalometric radiographs were obtained in different positions and the PSG signals were observed.Results:The improvements were excellent and the air ways were enlarged by combined UPPP and genioglossus advancement and hyoid suspension in the patients with severe OSAS.（Conclusion:That） different operation methods were chosen for aiming at different degrees and obstructive sites in the patients with OSAS was reasonable and efficacious.
