氦-氧混合气治疗慢性阻塞性肺疾患并高二氧化碳血症的临床应用

Heliox breathing for patients and complicated with hypercapnia in COPD

  • 摘要: 目的:观察氦-氧混合气治疗慢性阻塞性肺疾患(COPD)合并高二氧化碳血症的临床疗效。方法:选择17例慢性阻塞性肺疾患合并高二氧化碳血症患者,随机分为实验组(8例,吸入He-O2)和对照组(9例,吸入N2-O2),通过自制重复利用氦气的循环式氦-氧混合气辅助呼吸装置进行呼吸,实验中患者依次吸入FiO2分别为21%、40%、60%、80%的氦-氧(实验组)或空-氧(对照组)混合气,观察肺功能及血气的变化。结果:氦-氧混合气可以降低吸气负荷,促进氧合及体内二氧化碳排除;起效迅速,但后续效应短暂;氦-氧混合气的理想治疗浓度为40%~79%氦。结论:临床初步应用证明吸入氦-氧混合气治疗慢性阻塞性肺疾患合并高二氧化碳血症患者具有良好的疗效,循环式氦-氧混合气辅助呼吸装置可以显著节省氦气的消耗

     

    Abstract: Objective: To evaluate the effects of breathing helium oxygen (heliox) mixtures on COPD patients suffering from hypercapnia. Methods: Seventeen COPD patients were randomly divided into treatment group (breathing heliox, 8 cases) and control group (breathing N2-O2, 9 cases), and the FiO2 were in order of 21% O2, 40% O2, 60% O2 and 80% O2 respectively. Results: The data of pulmonary function tests, arterial blood gas values of the patients were recorded. The results showed that heliox reduced the breathing load, ameliorated oxygen diffusion and CO2 eliminating obviously, but the effective duration was very short after stopping of heliox breathing. The range of heliox effective treating concentration was 40%~79% He.Conclusion:The study demonstrated that heliox breathing may be one of effective ways in severe COPD patients, and the assisted respirator of recycled helium using may save helium distinctly and maybe a useful instrument for pulmonary obstructive diseases.
