降钙素原与血培养诊断血流感染比较

Procalcitonin and blood culture in diagnosis of blood stream infection

  • 摘要: 目的 比较血培养与血清降钙素原(Procalcitonin,PCT)对血流感染的诊断作用。方法 对我院2008年5月-2009年7月同时进行血培养与降钙素原检测患者资料进行回顾性分析,应用全自动连续性检测系统BacT/Alert3D进行血培养,VETEKⅡ微生物全自动鉴定仪对病原菌进行鉴定,半定量免疫色谱法检测降钙素原。结果 PCT和血培养的阳性率分别为23.8%和22.9%;PCT诊断的敏感性为78.3%、特异性为77.3%,阳性预测值和阴性预测值分别为52%、91.9%。结论 半定量PCT检测具有一定的敏感性和特异性,操作简单,容易判读,可用于常规实验检测。

     

    Abstract: Objective To compare the role of procalcitonin(PCT) and blood culture in diagnosis of blood stream infection. Methods Data about patients with blood stream infection diagnosed with PCT and blood culture in our hospital from May 2008 to July 2009 were retrospectively analyzed.Blood was cultured in BacT/Alert 3D,a fully automatic consecutive detection system.Pathogens were identified using VETEKII,a fully automatic microorganism identifier.Semi-quantitative immunochromatography was used to detect PCT. Results The positive rate of PCT and blood culture was 23.8% and 22.9%,respectively.The sensitivity and specificity of PCT were 78.3% and 77.3%,respectively,with a positive and negative predictive value of 52% and 91.9%,respectively. Conclusion Semi-quantitative PCT has a certain sensitivity and specificity in diagnosis of blood stream infection,and is easy to perform and judge,and can thus be used in routine laboratory diagnosis of blood stream infection.
