出院带药处方质量调查分析

Survey and analysis on the prescriptions for hospital discharge patients

摘要: 目的:分析出院带药处方的规范化和质量问题,提出改正措施。方法:抽取2006年8月-2008年8月全部出院带药处方37121张进行统计分析。结果:差错处方共3 906张,差错率为10.52%,涉及药物差错419张,占1.13%,书写规范化差错3406张,占差错处方的87.7%。结论:临床医师和药师均应加强《处方管理办法》和专业学习,提高开方和审方能力。

Abstract: Objective: To investigate and analyze the errors in the prescription of hospital discharge patients and to put forword the corresponding measures.Methods: From August 2006 to August 2008,a total of 37 121 prescriptions of leaving hospital patients were taken for the survey and analysis of errors.Results: In the total prescriptions,3 906（10.52%）were irrational,in which 419（1.13%）were related with drug.The main problems were the standardization of the prescription,3 406（87.7%） were erroneous in these way.Conclusion: Clinicians and pharmacist should make more efforts to learn"prescription management method" and expertise and to enhance the ability of writing and checking prescription.
