肝动脉碘油栓塞加抗癌药治疗原发性肝癌近期死亡原因分析及适应证的探讨

Analysis of causes of death by transcatheter arterial embolization using iodized oil and anti-cancer drugs for the treatment of primary liver cancer and study on the indication

摘要: 用肝动脉碘油栓塞加抗癌药丝裂霉素、阿霉素治疗103例原发性肝癌患者,近期(3个月内)死亡11例。分析死因认为术前高度黄疸,肝性糖尿病、患有原发性腹膜炎、肺间质感染并反复发作,有动静脉瘘的患者都不适宜作此疗法。治疗间隔期太长,会导致疗效不佳。为此,修改了适应证范畴。

Abstract: Transcatheter arterial iodized oil embolization combined with anticancer drugs was used to treat 103 cases of primary liver cancer. 11 cases died in the third month after this therapy.
According to the analysis of the causes of death, it seems that bilirubinemia, hepatic diabetes, spontaneous peritonitis, interstitial pneumionia, pulmonery embolism due to A-V fistula are contraindications for this therapy. If the therapatic intervention is prolonged, the prognosis is even worse. Depending on the clinical studies, it appears that indication for this therapy should always be considered.
