帕金森病患者苍白球神经元放电特性的离线分析

Off-line analysis of single-unit pallidal discharge of Parkinson’s disease

  • 摘要: 目的: 寻找鉴别Gpi与Gpe神经元放电指标。方法: 在内侧苍白球腹后部毁损术中,微电极记录苍白球及其周围结构的神经元放电,术后计算内侧苍白球(Gpi)和外侧苍白球(Gpe)神经元放电频率(f)、爆发式放电指数(BI)、间歇指数(PI)和间歇系数(PR)。比较Gpi和Gpe的f、BI、PI和PR的差异,计算分别以他们为指标时的灵敏度(Se)、特异度(Sp)、假阳性率(α)、假阴性率(β)和准确度(Ac)。结果: Gpi神经元f、BI、PI、PR分别为96± 43Hz、2.31± 1.81、0.05± 0.05、0.27± 0.28。Gpe神经元f、BI、PI、PR分别为5 9± 27、0.88± 0.63、0.20± 0.14、1.5 4± 1.17。f、BI、PI和PR为指标时鉴别Gpi和Gpe神经元时的Se、Sp、α、β和Ac分别为0.84、0.78、0.77和0.93;0.64、0.79、0.88和0.87;0.36、0.21、0.12和0.13;0.16、0.22、0.23和0.07;0.72、0.79、0.80和0.90。结论: PR是鉴别Gpi和Gpe神经元较好指标。

     

    Abstract: objective:to build an objective principle to distinguish the difference of the discharge patterns between Gpi and Gpe. Methods: The discharge of pallidum neurons is recorded and stored. The spike is demarcated after operation to compute the frequency (f) of the single-unit discharge, Bust Index (BI), Pause Index (PI) and Pause Ratio (PR). It is compared between the f,BI, PI and PR of Gpi and of Gpe. Compute Sensitivity(Se), Specificity(Sp), mistake diagnosis(α), omission diagnosis(β), and Accuracy(Ac) when f、BI、PI and PR are used as guideline respectively to differentiate discharge patterns between Gpi and Gpe. Results: The f、BI、PI、PR of Gpi is 96±43Hz、2.31±1.81、0.05±0.05、0.27±0.28 respectively. The f、BI、PI、PR of Gpe is 59±27、0.88±0.63、0.20±0.14、1.54±1.17 respectively. When f、BI、PI and PR is used as the index respectively to differentiate Gpi and Gpe neuron, the Se、Sp、α、βand Ac is 0.84、0.78、0.77 and 0.93;0.64、0.79、0.88 and 0.87;0.36、 0.21、0.12 and 0.13;0.16、0.22、0.23 and 0.07;0.72、0.79、0.80 and 0.90. Conclusion: PR is a better index to distinguish Gpi and Gpe.
