输液温度对失血性休克肾功能的影响

Effects of fluid temperature on hemorrhagic shock rabbit kidney function

  • 摘要: 目的: 观察输注不同温度的溶液后失血性休克复苏模型兔肾功能变化的特点,并探讨对减轻肾功能损害的作用。方法: 选择健康新西兰雄兔30只,随机分为4组进行手术插管及制作休克模型,在休克模型稳定30min后按相同速度相同液量分别给予低温(10.7± 1.6)℃、常温(20.6± 1.3)℃及温热(39.5± 1.3)℃平衡液和自体血;在休克前、休克及液体复苏1h、2h和4h监测血尿素、肌酐水平及尿量和尿比重的变化。结果: 复苏开始后各组尿量、肌酐、尿素及尿比重均有改善,尿量增加,肌酐、尿素及尿比重下降,其中以温热组效果较显著。结论: 液体复苏有助于改善肾功能,温热溶液复苏效果最佳。

     

    Abstract: Objective: To observe the change of hepatic function of the rabbit hemorrhagic shock and resuscitation model by infusing the different temperature fluids. Methods: After the rabbit hemorrhagic shock model was successfully duplicated, the 30 rabbits were randomized to four groups, receiving hypothermic fluid(10.7±1.6)℃, normothermic fluid(20.6±1.3)℃and hyperthermic fluid(39.5±1.3)℃.The urine volume, urine specific gravity, Cr and urea were examined at baseline, shock and resuscitation phases(1 h,2 h,4 h). Results: fluids infusions are propitious to the improve of renal function. The hyperthermic is the best choose. Conclusion: hemorrhagic shock may result in abnormal renal function. The infusion of hyperthermic fluids can more effectively improve renal function than normothermic fluids and hypothermic fluids.
