控制性低中心静脉压对肝切除手术患者心率变异性和血流动力学的影响

Effect of controlled low central venous pressure on heart rate variability and hemodynamics of patients during hepatectomy

  • 摘要: 目的 探讨在肝切除手术中采用控制性低中心静脉压技术对自主神经系统和血流动力学的影响。 方法 ASAⅠ-Ⅱ级29例患者在静吸复合全麻下择期行肝切除手术,术中采用控制性低中心静脉压技术。分别在诱导前、降压前、降压稳定即刻,降压后5、10、15min,复压稳定即刻,复压后5、10、15min,记录低频(LF),高频(HF),LF/HF比值,HR、MBP和双频谱指数(BIS)的值。 结果 LF/HF在复压稳定后10min较基础值升高(P<0.05),其他时间点和基础值相比无统计学差异(P>0.05)。HF在降压稳定即刻、降压后5-15min较降压前下降(P<0.05),LF/HF在降压稳定后10min较降压前增加(P<0.05),降压稳定后10min和15min较降压稳定即刻增加(P<0.05)。HF在复压后10min较复压稳定即刻下降(P<0.05),LF/HF在复压稳定后10-15min较复压稳定即刻增加(P<0.05);HR在降压稳定后10min较降压前升高(P<0.05),MBP在各个时间点较基础值下降(P<0.05)。 结论 在静吸复合麻醉下行肝切除手术中采用控制性低中心静脉压技术,中心静脉压在降复压过程中的变化对自主神经系统无明显影响,交感-迷走神经张力基本处于动态平衡,且血压和平均动脉压维持平稳。

     

    Abstract: Objective To study the effect of controlled low central venous pressure on heart rate variability(HRV) and hemodynamics of patients during hepatectomy. Methods Twenty-nine ASAⅠ-Ⅱpatients underwent elective hepatectomy with controlled low central venous pressure technique under venous-inhalation general anesthesia.Their MBP,HR,BIS,HF,LF,and HF/LF ratio were recorded before anesthesia(T0) and before deliberate hypotension(T1),Immediately(T2),at 5min(T3),10min(T4) and 15min(T5) after CVP <5mmHg(1mmHg=0.133kPa),and immediately(T6),at 5min(T7),10min(T8) and 15min(T9) after CVP>5mmHg. Results The LF/HF ratio was higher at 10min after pressure restore than its basic value(P<0.05).However,no significant difference was found in the LF/HF ratio and its basic value at the other time points.The HF was lower at T8 than at T6(P<0.05).The LF/HF was higher at T8-T9 than at T6(P<0.05).The HR was higher at T4 than at T0(P<0.05).The mean blood pressure was lower at T1-T9 than at T0(P<0.05). Conclusion Venous-inhalation general anesthesia exerts no significant effect on the autonomic nervous system of patients during hepatectomy with controlled low central venous pressure technique under venous-inhalation general anesthesia and can maintain their normal sympathetic and parasympathetic tone,blood pressure and mean blood pressure.
