单纯冠心病患者冠脉病变程度与生化指标相关性分析

Correlation between coronary lesion and biochemical parameters in patients with simple coronary heart disease

  • 摘要: 目的 探讨与单纯冠心病冠脉病变程度相关的生化指标。 方法 选取临床疑诊冠心病并除外有关疾病患者194例,根据冠脉造影结果分为冠心病组(CHD组)及对照组(NCHD组),CHD组根据冠脉病变支数细分为单支病变组(CHD1组)、多支病变组(CHD2组),观察各组间生化指标差异。 结果 CHD组血清脑利钠肽前体(N-terminal brain natriuretic peptide,NTBNP)、血清脂蛋白(a)lipoprotein a,Lp (a)水平明显高于NCHD组(P< 0.01),并在冠脉病变支数分组间两两比较差异有统计学意义(P< 0.05)。CHD组与NCHD组相比,心肌肌钙蛋白T (TNT)、葡萄糖(GLU)、血清尿素(BUN)、肌酐(CRE)增高(P< 0.05),高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(Apo-A1)降低(P< 0.01),并且与冠脉病变程度相关。 结论 除外糖尿病,肝、肾疾病及其他有关疾病的单纯冠心病患者仍存在GLU、BUN、CRE的代谢压力,NTBNP及TNT与单纯冠心病的早期病变相关,Apo-A1、HDL-C为冠心病的保护因子。

     

    Abstract: Objective To study the correlation between coronary lesion and biochemical parameters in patients with simple coronary heart disease (CHD). Methods One hundred and ninety-four patients with suspected CHD but without other diseases were divided into CHD group and control group according to their coronary angiography. CHD group was further divided into single coronary lesion group(CHD 1 group) and multi-coronary disease group (CHD 2 group) according to its coronary lesions. Differences in biochemical parameters were compared between the two groups. Results The serum levels of N-terminal brain natriuretic peptide(NTBNP), lipoprotein a(Lpa), cardiac troponin T(TNT), glucose(GLU), urea nitrogen(BUN), creatinine(CRE) were significantly higher while the serum levels of apolipoprotein-A1(Apo-A1) and high density lipoprotein cholesterol(HDL-C) were significantly lower in CHD group than in control group(P< 0.01, P< 0.05), which were positively related with the coronary lesions. Conclusion The metabolism of serum GLU, BUN, CRE changes in patients with simple CHD but without hepatic and kidney disease, diabetes and other diseases. The serum NTBNP and TNT are correlated with early CHD. Apo-A1 and HDL-C are the protection factors for CHD.
