扩张型心肌病患者左心室涡流特征研究

Observation of characterization and quantification of vortex flow in left ventricle of patients with dilated cardiomyopathy using vector flow mapping

  • 摘要: 目的 运用血流向量成像技术(vector flow mapping,VFM)评价扩张型心肌病(dilated cardiomyopathy,DCM)患者左心室涡流特征,探索临床评价心功能的新方法。 方法 选取健康志愿者40例和DCM患者20例作为研究对象,行常规超声心动图和VFM检查,采集图像后进行脱机分析,观察左心室涡流的演变规律,定量评估涡流各项参数,并进行统计学分析。 结果 与对照组相比,DCM患者涡流形态和位置变化更多,最大流量(Qmax)增大(P< 0.05),半值面积(S)及半径(r)明显增大(P< 0.01),而涡流的涡强(Qmax/S)显著减小(24.21±10.62 vs 15.27±7.92,P< 0.01),涡强与左室射血分数呈正相关(r=0.417,P=0.000)。 结论 DCM患者左室涡流的演变规律与正常状态大致相同,但涡流明显增大;舒张期涡流的涡强可能成为评价左心收缩功能的新指标。

     

    Abstract: Objective To assess the characteristics of left ventricle vortex flow in patients with dilated cardiomyopathy (DCM) using the vectorial flow imaging(VFM). Methods Forty healthy persons served as a control group and 20 DCM patients served as study group in this study. They underwent conventional echocardiography and VFM. Their images were analyzed. Evolution rules of their left ventricle vortex flow were observed. Different vortex flow parameters were quantitatively assessed and analyzed. Results More changes of vortex flow morphology and locations occurred in study group than in control group. The Qmax, semi radius area (S)and radius (s) of vortex flow were significantly larger while its Qmax/S was significantly lower in study group than in control group(15.27±7.92 vs 24.21±10.62, P< 0.01). The Qmax/S was positively related with LVEF (r=0.417, P=0.000). Conclusion The evolution rules of left ventricle vortex flow are similar between DCM patients and normal controls, but the vortex flow is greater in DCM patients than in controls. Qmax/S may be used as a new index for the assessment of left ventricular systolic function.
