三维斑点追踪成像技术评价2型糖尿病患者左心室收缩功能

Application of 3-dimensional speckle tracking imaging in assessing left ventricular systolic function of patients with type 2 diabetes mellitus

  • 摘要: 目的 应用三维超声斑点追踪成像技术 (3D-STI) 评价2型糖尿病患者左心室收缩功能。 方法 2型糖尿病患者40例, 正常志愿者40例。采集标准心尖四腔切面进行左心室全容积图像并存储, 应用3D-STI软件进行在线分析, 自动计算出左心室舒张末期容积 (left ventricular end-diastolic volume, LVEDV) , 左心室收缩末期容积 (left ventricular end-systolic volume, LVESV) , 左心室射血分数 (left ventricular ejection fraction, LVEF) , 以及左心室整体纵向收缩期峰值应变 (left ventricular global longitudinal peak systolic strain, LVGLS) 、左心室整体径向收缩期峰值应变 (left ventricular global radial peak systolic strain, LVGRS) 、左心室整体圆周收缩期峰值应变 (left ventricular global circumferential peak systolic strain, LVGCS) 、左心室整体面积收缩期峰值应变 (left ventricular global area peak systolic strainLVGAS) , 并比较糖尿组与正常对照组各指标的变化情况及与射血分数之间的相关性。 结果 糖尿病组LVEDV、LVESV较正常组增加, LVEF较正常对照组减低 (P均<0.05) ;糖尿病组LVGLS较正常对照组显著减低 (P<0.01) , 糖尿病组LVGRS、LVGCS及LVGAS较正常对照组减低 (P均<0.05) ;两组LVGLS、LVGRS、LVGCS及LVGAS与LVEF均显著相关。 结论 三维斑点追踪成像技术可较准确地评价左心室整体收缩功能。

     

    Abstract: Objective To assess the left ventricular systolic function of type 2 diabetes mellitus(DM) patients with 3-dimensional ultrasound speckle tracking imaging(3D-STI). Methods Forty type 2 DM patients served as a DM group and 40 healthy volunteers served as a control group in this study.3D-STI was recorded from standard left ventricular apical 4-chamber views.Left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV),left ventricular ejection fraction(LVEF),and left ventricular global longitudinal peak systolic strain(LVGLS),left ventricular global radial peak systolic strain(LVGRS),left ventricular global circumferential peak systolic strain(LVGCS),left ventricular global area peak systolic strain(LVGAS) in the two groups were measured and compared.Their correlation was analyzed. Results The LVEDV,LVESV were higher in diabetes groups than in normal controls,and the LVEF were lower in diabetes groups than in normal controls(P<0.05);The LVGLS were significantly lower in diabetes groups than in normal controls(P<0.01),The LVGRS,LVGCS and LVGAS were lower in diabetes groups than in normal controls(P<0.05);The LVGLS,LVGRS,LVGCS and LVGAS were significantly correlated with LVEF in type 2 diabetes mellitus patients and normal controls. Conclusion 3D-STI can accurately assess the left ventricular global systolic function of type 2 DM patients.
